Record an Outcome for an Intervention
- From the navigation bar, click Care Plan.
The Care Plan template appears.
- Navigate to the Goals, Interventions and Outcomes panel.
The Select a Health concern to document grid is populated with all previously identified health concerns.
- Select a health concern from the list.
The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right.
- Select the goal that you are adding an intervention outcome for.
The data for the selected goal populates the data entry fields to the right.
- Click the Interventions tab.
The full Interventions tab appears with the previously entered interventions in the Select an Intervention to document grid.
- Click the intervention in the grid that you are reporting an outcome for.
- Click the Outcomes tab.
The full Outcomes tab appears with the data entry fields for the outcome to the right.
- Click the Outcome field and type a description of the outcome.
- Click the Outcome date field and select the date that the current outcome was achieved.
- Select the appropriate option for the Goal achieved field:
- If the goal has been achieved, click Yes.
- If the goal has not been achieved, click No.
- If the goal cannot be achieved, click Goal not attainable.
- If there is no progress towards achieving the goal, click No progress toward goal.
- Click the associated text box and select a progress description from the list.
To enter a description not on the list, click the blank value at the top of the list and type a description in the text box.
Note: The value selected for the Goal achieved field in the Goals tab will be synchronized with the value selected in the Outcome tab and vice versa. - Click Add.
The system adds the outcome to the Select an Outcome to document grid.
