Update an Intervention Outcome

You can modify all fields for an outcome entered during the current encounter. Since outcomes are encounter specific, you cannot view outcomes entered during previous encounters in the Goals, Interventions and Outcomes panel and therefore cannot modify those outcomes. You can view outcomes from previous encounter in the Care Plan History panel. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal for which you what to update an intervention outcome. The system populates the fields to the right of the grid with the data from the selected record. Click the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be updated. Click the Outcomes tab. Record an Outcome for an Intervention

From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list. The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention outcome for. The data for the selected goal populates the data entry fields to the right. Click the Interventions tab. The full Interventions tab appears with the previously entered interventions in the Select an Intervention to document grid. Click the intervention in the grid that you are reporting an outcome for. Click the Outcomes tab. The full Outcomes tab appears with the data entry fields for the outcome to the right. Remove a Goal Intervention

You can remove an intervention from the current encounter without restriction. You can also remove interventions from prior encounters, but you cannot remove an intervention from an earlier encounter if outcome data has been entered for that intervention during a subsequent encounter. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal in the left grid that has an intervention to be removed. The system populates the fields to the right of the grid with the data from the selected record. Remove a Health Concern Goal

You can only remove a health concern goal from the encounter where the latest intervention or outcome was recorded. When you remove a goal, the system removes all associated interventions and outcomes. Open the patient encounter where the latest intervention or outcome for the goal to be removed was recorded. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal to be updated in the left grid. The system populates the fields to the right of the grid with the data from the selected record. Click Remove. The system removes the goal and all related interventions and outcomes. Update a Goal Intervention

If no outcome has been entered, you can update all Intervention data fields for the current encounter. If an Outcome has been entered, you cannot update the Category field and the related data field. If updating interventions entered during earlier encounters, you may only be able to update some of the fields. You receive an alert to that effect. To update those fields, you must return to the earlier encounter. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Complete an Intervention

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. When the goal of an intervention has been achieved, you can complete the intervention. Completing the intervention removes it from the Interventions grid. On the Plan/Intervention tab, expand the Interventions panel. Select an entry on the Interdisciplinary Team Member column. The system displays the interventions designated for the selected team member type in the grid. Select the intervention that you want to update and click Edit. The Nursing Care Plan Data template appears. The template is populated with the data for the selected intervention. Select the Intervention complete option. Change any other enabled fields as needed. Click Save & Close. The intervention is completed and removed from the intervention panel. Parent topic: Interventions Panel
