Update an Intervention

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. The interventions appear only if they are added in the Nursing Care Plan Data template in the History panel. On the Plan/Intervention tab, expand the Interventions panel. Select an appropriate entry from the Interdisciplinary Team Member column. The system displays the interventions designated for the selected team member type. Interventions Select the intervention that you want to update and click Add. The Nursing Care Plan Data template opens. The template is populated with the data for the selected intervention. Nursing Care Plan Data Note: The system populates the Team Member field with the name of the current user and the Intervention frequency field with the currently defined intervention frequency. Create a Task for an Intervention

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. If an intervention requires an action by a provider, you can create a task for that provider. The tasks you create are not linked to a particular intervention and behave as any other task in the tasking function. On the Plan/Intervention tab, expand the Interventions panel. Select the intervention for which you need to create a task and click Task. The To Do window appears. To Do The Date Due and Priority fields are pre-filled with current date and priority. Note: If you want to specify a different date or priority, click the corresponding fields and then select appropriate value. Select a Subject and a Category. Type the message in the Message field. Click Save & Close. The Attach template appears. Attach Do one of the following: If you do not want to attach a document, then click the No Attachment option. Interventions Panel

When the goal of an intervention has been achieved, you can complete the intervention. Completing the intervention removes it from the Interventions grid. The entry for the intervention on the History grid is updated with Complete in the Status column. Remove a Goal Intervention

You can remove an intervention from the current encounter without restriction. You can also remove interventions from prior encounters, but you cannot remove an intervention from an earlier encounter if outcome data has been entered for that intervention during a subsequent encounter. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal in the left grid that has an intervention to be removed. The system populates the fields to the right of the grid with the data from the selected record. Remove a Document Entry

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Plan/Intervention tab, expand the Document panel. Click the entry on the grid to be removed. Remove an Intervention Outcome

You can remove an outcome only in the encounter where the latest outcome was documented for that intervention. Open the patient encounter where the latest outcome for the intervention was documented. From the navigation bar, click Care Plan. The Care Plan template appears. Navigate to the Goals, Interventions and Outcomes panel. Click the goal for which you what to remove an intervention outcome. The system populates the fields to the right of the grid with the data from the selected record. Click the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be removed. Click the Outcomes tab. The full Outcomes tab appears with the data for the selected intervention. Select the outcome to be removed from the grid. Click Remove. The system removes the outcome.
