Record Skin Cancer History

  1. Open the Histories - Dermatology template.
  2. In the Skin Cancer Log panel, in the Treatment History section, click Add.
    The Skin Cancer History Log - Derm template opens.

  3. In the Treated? section, select the N option.
  4. Type the year the cancer was diagnosed in the Year of DX field.
  5. To select the characteristics of cancer as reported on the pathology report, click Characteristics.
  6. In the Accession field , type the accession number from the pathology report.
  7. Select the correct specimen id from the Specimen ID list.
  8. To open the Diagnosis template, click Diagnosis, and then locate and select the diagnosis as reported on the pathology report.
  9. Select the location where the patient is identified with cancer in the Location field.
  10. Select the treatment needed for this patient in the Recommended treatment field.
  11. Type a description of any problems associated with the report, the specimen or the patient in the Problems field.
  12. Click Save & Close.
    You are returned to the Histories template.

    The application places a record for skin cancer in the Treatment History grid. In addition, an Untreated Skin Cancer alert appears on all the templates and remains until you report that the skin cancer has been treated.
