Record a Risk Assessment

Any assessment of risk data that was entered previously on the Comprehensive Assessment template set carries forward to the panels found here. On the Risk Assessment panel, if the patient is not a danger to self or other, select the option for Patient is not currently a danger to self, others, or property. Selecting this option sets every field in the section as unavailable. The Encounter field fills with the current date. In the Risk field, select the appropriate type of risk from the list. If there is a need to record a previous risk for the client, select the Past option. CAUTION: Always document past risks before a current assessment. If Past is selected, either an Event Date (even if approximate) or Denied should be populated. If there are no past assessments to document, select the option for Current. Select Denied or Admits. If Denied is selected, the documentation options are all disabled in this section except for the date fields and the Add and Clear buttons. Enter the Event

Enter Allergy Data on Psychiatric Evaluation

On the BH - Psychiatric Evaluation template, expand the Medication Information panel. In the Allergies section, click the Comments link to include relevant details. Select No known allergies, Reviewed updated, or Reviewed, no change. Click Add or Update to enter data on the Adult Allergies or Pediatrics Allergies template. Note: The severity of the reaction cannot be added on the Allergies panel. Go to the Medication Allergies module to add the severity of the reaction, where multiple reactions and severities can be selected. After the severity is added in the Medication Allergies module and updated, the severity appears in the Allergies panel grid under the Reaction (Severity) column. Enter Medication Information

In the Active section on the Medication Information/Consent panel, select the Comments link to include relevant details. To enter medications, select Add/Update. The Medications module opens. Any medications you enter display in the medications grid on the Psych Eval (BH) template. To reconcile medications, select Reconcile. The Medication Review window opens and you can complete the reconciliation process. In the Active Medications grid, highlight a medication. The medication details display in the Medication field in the Medications Consent section. In the Target Symptoms for Consent field, enter text. In the How it was discussed field, select a method from the list. Enter the Patient/Guardian Initials and the Provider Initials. Select Add. The details display in the grid. In the Other Medications Reported by Patient and Comments on past medications fields, document medications not prescribed by the prescriber or facility. Document Medication Information

On the BH - Psychopharm template, expand the Medication Information panel. Answer the following questions with Yes, No, or Not applicable as applies: Takes meds as prescribed Changes in medical status Other meds Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open the Medication Side Effects - BH template. Entering information on the template automatically selects the Yes option in the Medication Information panel and turns the Side effects reported link purple to indicate that data is present from that link. At the top of the grid, click the Comments link if additional information is required. All prescriptions currently existing in the Medications module are visible in the grid under the Active Medications section. If a medication is Inactive, a stop date appears in the grid. Enter the Client's Current Prescription Information

On the Medication information panel, if the client is not on any medications and the grid is empty, select the None reported option. In the Active and inactive medications section, click in the grid to open the Medications module. When a row is populated, highlight it and click the Patient Education button to print out educational material associated with the selected prescription. To include medication details in the generated document, click Comments. Under Other medications Reported by Patient, use the Add, Update, Remove, and/or Clear buttons to enter the correct information into the grid. Note: Non-clinical staff may be entering this information and may not be clear as to exactly what additional medications the client may be taking. Additionally, some clients may claim that they are on medications but deny that they have the condition for which the medication is given. Document Medication Information

On the BH - Nursing Note template, navigate to the Medication Information panel and answer the following questions with No, Yes, or N/A (Not Applicable) and enter Comments as needed: Takes medications as prescribed Changes in medical status Reviewed medication name(s), dosage, purpose, and frequency Were medications delivered today - If yes, for what duration Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open a template on which medication side effects can be documented. Next to Other medications, select from the following options: None Herbal Over the counter Other - and type specifics in the associated field Click the Office Procedures link to launch the My Plan tabbed template set on the Office Procedures tab and document medications administered today.
