Record a Risk Assessment

Any assessment of risk data that was entered previously on the Comprehensive Assessment template set carries forward to the panels found here.

  1. On the Risk Assessment panel, if the patient is not a danger to self or other, select the option for Patient is not currently a danger to self, others, or property.
    Selecting this option sets every field in the section as unavailable. The Encounter field fills with the current date.
  2. In the Risk field, select the appropriate type of risk from the list.
  3. If there is a need to record a previous risk for the client, select the Past option.
  4. If there are no past assessments to document, select the option for Current.
  5. Select Denied or Admits.
    If Denied is selected, the documentation options are all disabled in this section except for the date fields and the Add and Clear buttons.
  6. Enter the Event Date or if the exact date cannot be established, select This is approximated date.
  7. Select No or Yes for Ideation and Plan
  8. Select No or Yes for Intent and if Yes, select a rating of 1-10 from the Intent scale picklist.
  9. Select No or Yes for Attempt and if No, the remaining questions are disabled.
  10. Select Planned or Impulsive for Was it planned or impulsive.
  11. Select No or Yes for the remaining questions.
  12. Enter details under Describe plan/attempt field and click Add.
    The grid view defaults to This encounter.
  13. In the Safety Management Plan field, type up to 500 characters to indicate how the client protects themselves and/or others from the issues identified in the Risk Assessment.
