Document Risk Assessment
- Open a risk assessment template, and expand the Risk Assessment panel.
- To document a potential for suicide and homicide risk, select one or more of the following links:
- Suicidal/Homicidal Risk
- Safety Risk
- Columbia Suicidal Severity Rating Scale (C-SSRS)
- SAFE-T Protocol with C-SSRS (Columbia Risk and Protective Factors)
Note: Only one of the C-SSRS template appears based on the configuration in the BH Foundation Configuration template. - To open the risk history view, select the Risk Assessment History link.
The Risk Assessment History window opens.
- If the client is not currently and has never been a danger to themselves, others, or property, select the respective checkbox. If selected, the remaining fields are turned off.
- In the Risk field, select a risk type.
- Select if the risk is current or from the past.
- Select if the patient admits or denies the risk event.
Note: If you select Denied, the remaining fields are disabled.
- Enter the event date. If applicable, select This is approximated date.
- Enter the remaining risk details.
- In Describe plan/attempt, enter additional details.
- Select Add.
The documented details appear in the grid.
- In the Safety Management Plan field, enter the plan developed for the client.
Note: The Safety Management Plan field is also populated by clinical data from previous encounters. To clear the data pulled forward from previous encounters, select Clear Pull Forward History.
