Document Risk Assessment

  1. Open a risk assessment template, and expand the Risk Assessment panel.
  2. To document a potential for suicide and homicide risk, select one or more of the following links:
    • Suicidal/Homicidal Risk
    • Safety Risk
    • Columbia Suicidal Severity Rating Scale (C-SSRS)
    • SAFE-T Protocol with C-SSRS (Columbia Risk and Protective Factors)
  3. To open the risk history view, select the Risk Assessment History link.
    The Risk Assessment History window opens.

  4. If the client is not currently and has never been a danger to themselves, others, or property, select the respective checkbox. If selected, the remaining fields are turned off.
  5. In the Risk field, select a risk type.

  6. Select if the risk is current or from the past.
  7. Select if the patient admits or denies the risk event.
  8. Enter the event date. If applicable, select This is approximated date.
  9. Enter the remaining risk details.
  10. In Describe plan/attempt, enter additional details.
  11. Select Add.
    The documented details appear in the grid.

  12. In the Safety Management Plan field, enter the plan developed for the client.
