Document the Patient's Initial Psychiatric Evaluation Summary

As a psychiatrist, nurse practitioner (NPP), or other behavioral health professional, you can document an initial psychiatric assessment of the patient. It is important to note that you can prescribe medications during the psychiatric evaluation. Also, the EM Coding button opens the Finalize template, which enables you to submit billing codes. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Psych Eval (BH), and then select OK. Note: You can open the Psych Eval (BH) template from your Packet Navigation, or from the Templates module. Record Medication Data

On the BH - Psychiatric Evaluation template, expand the Medication Information panel. In the Active Medications section, select a Patient status. If required, click the Quality button for information associated with the Clinical Quality Measure (CQM). Click the Comments link to include medication details in the generated document. Select one of these options: No medications this encounter Medications reconciled Click Add/Update or Reconcile to update the data in the grid. To include Other Medications Reported by Patient, use the Add, Update, Remove, or Clear buttons. CAUTION: It is highly recommended that the Other Medications Reported by Patient values are verified for accuracy before entering them into the grid for Active Medications. Document Medication Information

On the BH - Nursing Note template, navigate to the Medication Information panel and answer the following questions with No, Yes, or N/A (Not Applicable) and enter Comments as needed: Takes medications as prescribed Changes in medical status Reviewed medication name(s), dosage, purpose, and frequency Were medications delivered today - If yes, for what duration Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open a template on which medication side effects can be documented. Next to Other medications, select from the following options: None Herbal Over the counter Other - and type specifics in the associated field Click the Office Procedures link to launch the My Plan tabbed template set on the Office Procedures tab and document medications administered today. On the BH - Psychopharm template, expand the Medication Information panel. Answer the following questions with Yes, No, or Not applicable as applies: Takes meds as prescribed Changes in medical status Other meds Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open the Medication Side Effects - BH template. Entering information on the template automatically selects the Yes option in the Medication Information panel and turns the Side effects reported link purple to indicate that data is present from that link. At the top of the grid, click the Comments link if additional information is required. All prescriptions currently existing in the Medications module are visible in the grid under the Active Medications section. If a medication is Inactive, a stop date appears in the grid. Reconcile Medications Manually

Use the Medications Review panel on the Medications Review template to reconcile medications manually. Note: If no manual reconciliation has taken place, the Manual Reconciliation check box in the Reconciliation type panel is unchecked; as soon as you reconcile any medication manually, the box is selected and fields open for you to enter additional data. Medication Review template Click the Reconcile button below the Medications grid to open the Medication Review template. If the patient is taking all medications in the Medication List grid as directed, click the Review All – Taken As Directed button to move them all into the Medication Review grid. If not, skip to step 3. Select the check box Review – adherence at the top of the Medication Review panel. The drop-down field selection defaults to “taking as directed”. You can document and manage the medication history and current medications of a patient. You can use the Medications panel to view the patient's current medications and medication history, enter additional prescriptions for the patient, and reconcile the medications listed with the actual active medications for the patient. You can use the No medications this encounter check box if there are no active medications for the patient. Medications Panel You can reconcile the medications for the patient using the Medications reconciled check box, and indicate that you have received the summary of care for the patient using the Summary of care received check box.
