Document Individuals Present

In the Individuals Present panel, select from one of the Contact Type options. Select one of the following in the Individuals present section: Individual not present Individual present Select Others present and enter the details in the Support Resources section. In the Support Resources section, record the client's closest friends and relatives. To add the support resources data in the grid, click Add. To change the grid view, select All or Active. The Effective and End dates determine if a resource is Active. Enter the Prescriber's Evaluation

Expand the Prescriber's Evaluation panel. If there are no modifications, select the No changes reported/observed checkbox. In the Interim Update field, use the My Phrases link or manually type information about the client's progress since the last visit related to symptoms, effectiveness of medications, substance use, changes in family/social history, overall functioning, and any new issues. Record Plan Details

On the BH - Psychopharm template, navigate to the Plan panel. Document the Diagnosis since the last visit by selecting from the following: No change Yes, CA update required Next to Rationale for changing medications, select Not indicated or enter an explanation in the associated field. Answer No or Yes to: Were Medications delivered today. Next to If Yes, for what duration select one of the following options: Weekly Monthly Other Select No or Yes to indicate if there was a Prescription written. Document if the medication was Called into pharmacy. Next to Lab tests were ordered, select No or Yes. If Yes, enter available Results in the associated field Click the Review Labs link to open the Orders module and to view existing lab orders for this client. Document Instructions/comments/plan, as applicable. To schedule a follow up, click the Follow Up button to the right of Next Appointment. Enter Allergy Data

On the BH - Psychopharm template, expand the Medication Information panel. In the Allergies section, select No known allergies, Allergies added today, or Reviewed, no change. Click Add to add new allergies for the patient. The Adult Allergies or Pediatric Allergies template appears. Select the allergen reported by the patient. The Allergy Reaction list appears. Enter a Reaction from the list that automatically launches or click on the top blank row to type a reaction. Click Save & Close to close the template and enter the data into the grid. Select the Reviewed, updated option. Note: The severity of the reaction cannot be added on the Allergies panel. Go to the Medication Allergies module to add the severity of the reaction, where multiple reactions and severities can be selected. After the severity is added in the Medication Allergies module and updated, the severity appears in the Allergies panel grid under the Reaction (Severity) column. On the BH - Nursing Note template, navigate to the Medication Information panel and answer the following questions with No, Yes, or N/A (Not Applicable) and enter Comments as needed: Takes medications as prescribed Changes in medical status Reviewed medication name(s), dosage, purpose, and frequency Were medications delivered today - If yes, for what duration Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open a template on which medication side effects can be documented. Next to Other medications, select from the following options: None Herbal Over the counter Other - and type specifics in the associated field Click the Office Procedures link to launch the My Plan tabbed template set on the Office Procedures tab and document medications administered today. Enter Medication Information

In the Active section on the Medication Information/Consent panel, select the Comments link to include relevant details. To enter medications, select Add/Update. The Medications module opens. Any medications you enter display in the medications grid on the Psych Eval (BH) template. To reconcile medications, select Reconcile. The Medication Review window opens and you can complete the reconciliation process. In the Active Medications grid, highlight a medication. The medication details display in the Medication field in the Medications Consent section. In the Target Symptoms for Consent field, enter text. In the How it was discussed field, select a method from the list. Enter the Patient/Guardian Initials and the Provider Initials. Select Add. The details display in the grid.
