Documenting Vital Signs

You can document and update a patient's vital signs on the Vital Signs template. You can edit or remove a vital signs record from the Vital Signs grid. The vital signs template names vary based on a patient's age. For an adult patient, the template name is Adult Vital Signs. If a patient is younger than two years, the template name is Pediatric Under 2 Years Vital Signs. If a patient is two years or older, the template name is Pediatric Over 2 Years Vital Signs. You can change a pediatric patient's age to an adult age on the Practice Configuration template. A patient's height and weight fields are used to calculate their Blood Mass Index (BMI). BMI is not available for patients who are younger than two years. You can configure the normal vital signs range. If a patient's vital signs are outside the normal range, a message appears. Growth Chart Measurements

You can document measurements using the age-based Vital Signs templates for pediatric patients (vital_signs_peds_over2y and vital_signs_peds_under2y). You can access these templates in the following ways: From the Record Vital Signs template (VS_WA_Std). From different specialty Home and Office Visit templates. Directly from the Intake and SOAP tab templates of the Advanced template set and the Foundation template set. Note: You can customize settings for alerts for out-of-range pediatrics vital signs values using the ngkbm_vital_signs practice template. Premature growth charts are generated when the Gestational age at birth field has a value of less than 37 weeks and a gestational age of up to 50 weeks. You can enter the gestational age on the Pediatric Under 2 Years Vital Signs template or the Pregnancy/Birth History template. Generate Growth Charts on the Vital Signs Template

You can select the Growth Chart link on the Pediatric Vital Signs template to view a set of up to four growth charts at a time in a single window. The set may include growth charts for height, weight, head circumference, and BMI. Before you begin Before you generate a growth chart, you must set up the appropriate graphing preferences in System Administrator. Add Vital Signs of Pediatric Patients

You can document the vital signs of a pediatric patient on the Pediatric Vital Signs template. The template displays the weight for an age per length and sex-specific percentile value for patients who are younger than three years, based on their weight and height. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, click Templates to open the Select Template window, and then select the Intake template. In the Vital Signs panel, click Add. The Pediatric [Age] Vital Signs template opens. In the Height/length measurements section, do one of the following: To use data from a previous encounter, select Carried forward . To enter data from today's encounter, select a position and then enter the height, and then select Measured today. Note: If the value entered is not within the range, a message appears to indicate that the value entered is out of range. Document Vital Signs

You can document the vital signs of a patient from the Vital Signs panel of the Intake - Cardiology template. The vital signs template is based on a patient's age. For an adult patient, the template is Adult Vital Signs. For a patient who is younger than two years of age, the template is Pediatric Under 2 Years Vital Signs. For a patient who is two years or older, the template is Pediatric Over 2 Years Vital Signs. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template and click the Vital Signs panel. Click Add. The vital signs template opens depending on the age of the patient. Enter the following details on the vital signs template. Height/length measurements Weight measurement Temperature Blood Pressure and pulse Respiration and Pulse Ox Peak flow Pain level BMI/BSA calculation For a female patient, enter the Last Menstrual Period (LMP) details. Patient Vital Signs

You can document and manage a patient's vital signs. You can add various vital signs for a patient during an encounter. The most recent value for each vital sign appears in the grid. A number appears adjacent to the vital sign to indicate the number of times a value is recorded for the vital sign. You can point to the number to view the values and the time when each value is recorded. Vital Signs Section You can view only the vital sign records documented for the current day in the grid. To view the historical vital signs records, you can use the History link. To view the vital signs graphs you can enable the Graph link. If the vital sign interface is installed, you can use the Import and Refresh buttons to open the VitalSignsSpotMonitor interface. Based on the configuration done in the Vital Signs section on the Practice Configuration template, you can turn on the Import and Refresh buttons and manage the security groups for the alerts.
