Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. Adult Social History The Social History Review section enables you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History check box in the Social History section of the Practice Configuration practice template to set the template as default. Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template. Before you begin You must clear the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit. The Social History - Tobacco template opens. Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar. Document necessary details in the following panels on the Social History - Tobacco template: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter comments and select OK. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details. Document Tobacco Cessation Efforts

You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel. Select the Have you ever tried to quit using tobacco? option. If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link. Social History – Tobacco To add the details of the patient's efforts to quit tobacco, do the following: Select the tobacco type, and then enter the date the patient attempted to quit tobacco. Enter the longest time that the patient was tobacco free. You can enter number of days, weeks, months, or

The client's current smoking status must be recorded to meet Quality Reporting requirements. The Social History - Tobacco template in the Behavioral Health template set only includes the Tobacco option as the other social history data is documented in different sections of the template set. This ensures that users are documenting the required data and prevents duplicate information from printing in the documents. In the Substance Use/Addictive Behavior History panel, click the Tobacco Usage link. Select one of the following options for Have you ever used tobacco?: No/never Yes Unknown If you select No/never or Unknown, click Save & Close to return to your previous workflow. Select Yes or No for Have you used tobacco in the last 30 days? Select Yes or No for Have you used smokeless tobacco products in the last 30 days? Note: For the current encounter, if you select No\Never for Have you ever used tobacco?, the system selects No for both questions. If you have selected No/Never in the pr

Add Tobacco and Vaping Details

You can document the current tobacco use details and vaping details of a patient. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, select Tobacco/Vaping Use, and click Add/Edit. In the History Summary panel on the Intake template, click Social, click Tobacco/Vaping Use, and then click Add/Edit. Social History – Tobacco Expand the Tobacco Use panel. Note: For a patient whose age is 12 and above, the smoking status must be recorded to meet the Quality Reporting requirements. Select the appropriate options for the questions, and document the details in the necessary sections according to the tobacco usage by the patient. Document vaping details in the Vaping Use panel and enter any comments.
