Categories in Social History

You can use different sections in the Social history to document the history of a patient.

The following categories are available in the Social history.

CategoryDescription
Tobacco/Vaping UseEnables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details.
Alcohol/Caffeine

Enables you to document the alcohol and caffeine use for the patient.

Statuses/GeneralEnables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template.
LifestyleEnables you to document the lifestyle details. Includes sleep patterns, activities, exercise, religious information, home environment and safety, and recent travel.
OccupationEnables you to document details about the current employment and the employment status.
CommentEnables you to enter comments related to the patient's social information.
Diet HistoryEnables you to document the patient's diet details, including food intake and frequency.
EnvironmentalEnables you to document information about the patient's environment, including details about the patient's residence, pets, and hobbies.
