Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Document Vaping Details of a Patient

The Tobacco/Vaping Use panel in the Social History - Tobacco template enables you to screen and document the vaping details of a patient. The documented vaping details are carried forward to future encounters. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Vaping Use panel. In the Screened for vaping field, select Yes. Select a Status for vaping. If the patient is a current user, select a number in the Age started field. If the patient is not a current user, select a number in the Age started and Age stopped fields. Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template. Before you begin You must clear the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit. The Social History - Tobacco template opens. Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar.
