Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template.

1. Open NextGen® Enterprise EHR.
2. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit.
   The Social History - Tobacco template opens.

   Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar.
3. Document necessary details in the following panels on the Social History - Tobacco template:
   • Tobacco Use
   • Historical Use
   • Efforts To Quit Tobacco
   • Vaping Use
   • Passive Exposure
4. Enter comments and select OK.
5. Select another link from the left pane to document the social history of the patient.
6. Select OK.
   You can select the History Review link to open the History Review template.
