Document Tobacco Cessation Efforts

You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template.

  1. Open NextGen® Enterprise EHR.
  2. To open the Social History – Tobacco template, do one of the following:
    • On the Histories template, expand the Social panel, and select Add/Edit.
    • In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit.
  3. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel.
  4. Select the Have you ever tried to quit using tobacco? option.

    If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link.

  5. To add the details of the patient's efforts to quit tobacco, do the following:
    1. Select the tobacco type, and then enter the date the patient attempted to quit tobacco.
    2. Enter the longest time that the patient was tobacco free.
      You can enter number of days, weeks, months, or years.
    3. Enter the cessation method and reason for relapse.
  6. Select Add.
    The details appear in the grid.
  7. To document tobacco cessation information, select the Tobacco cessation discussed link.
  8. Select OK.
