Manage Environmental Information for a Patient

You can update or delete residence, animal, and miscellaneous details about the environmental factors that affect the patient on the Environmental template. Open NextGen® Enterprise EHR. In the Social panel, select Add/Edit. The Adult Social History template opens. Adult Social History In the Environmental panel, in the Residence section, do one of the following: To update a residence details, select the saved residence record in the grid, make the necessary changes, and select Update. To delete a residence, select the entry in the grid, and select Remove. In the Animal section, do one of the following: To update animal details, select an animal record saved to the grid, make the necessary changes, and select Update. Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template. Before you begin You must clear the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit. The Social History - Tobacco template opens. Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar. Document necessary details in the following panels on the Social History - Tobacco template: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter comments and select OK. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Select Save & Close. Categories in Social History You can use different sections in the Social history to document the history of a patient. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. Document the Patient's Status You can document the status of a patient on the Social History - Status template. Screen for Future Fall Risk You can document the patient's risk for falls. Document Occupation of a Patient

You can document the occupation details of a patient in the Social History - Occupation template. Open NextGen® Enterprise EHR. To open the Social History - Occupation template, do one of the following: On the Histories template, expand the Social panel, select Occupation, and then select Add. In the History Summary panel on Intake, select Occupation from Social, and then select Add. Enter the employment and employment status in the corresponding fields. Enter phone number and restrictions in the corresponding fields. Select occupation from the list in the Occupation field. To clear and select another occupation, select Cl button and select occupation from the list. To select the retirement date, select the Retired field. Save and close the template. Document a New Family History

You can add the family history of a patient from the Family category in the Family panel on the Histories template. Open NextGen® Enterprise EHR. Open the Histories template and then expand the Family panel. In the Family panel, select Add. The Family Health History template opens. Note: NextGen® Enterprise EHR fills Specialty field with the current specialty. The available conditions for the specialty appear alongside a check box on the Family Health History template. To indicate that the patient does not have any family history, select the No family history of check box. In the Relationship field, select a description from the Family relation list. In the Family member name field, enter the name of the selected family member, and do one of the following: If the family member does not have any condition that can be reported, select the Alive and well option.
