Add Environmental Information that Affect the Patient

Before you begin You must select the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Adult Social History template. Open NextGen® Enterprise EHR. In the Social panel, select Add/Edit. The Adult Social History template opens. Adult Social History In the Environmental panel, in the Residence section, do the following: To add information about the patient's primary residence, select 1 for Residence. Enter the appropriate residence information in the respective fields. Select Add. The residence information displays in the grid. Repeat the steps to add a second residence for the patient using the 2 option for Residence. Residence section In the Animals section, do the following: Animals section To document animal information, select 1 or 2 for Residence depending on which residence the animal is in. Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template. Before you begin You must clear the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit. The Social History - Tobacco template opens. Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar. Document necessary details in the following panels on the Social History - Tobacco template: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter comments and select OK. Manage Patient History Records

You can modify the patient history records. Open NextGen® Enterprise EHR. To update a patient history record, do the following: For the Past Medical/Surgical, Interim, Family, and Diagnostic categories, select the record from the grid, and update the necessary details. To edit a history record for the Social category, select the relevant option from the list, select Add/Edit, and then update the necessary details. To remove a patient history record, do the following: In the History Summary panel, for the Past Medical/Surgical, Interim, and Family categories, select the record from the grid, and select Remove. Note: You cannot delete the history records in the Social category. However, you can update the records. For the Social category, you must select the subcategories to review and update the social history details on the Social History template. For the Diagnostic category, select Add/Edit, select the record from the grid on the Diagnostic History Entry template, and select Delete.

Document Occupation of a Patient

You can document the occupation details of a patient in the Social History - Occupation template. Open NextGen® Enterprise EHR. To open the Social History - Occupation template, do one of the following: On the Histories template, expand the Social panel, select Occupation, and then select Add. In the History Summary panel on Intake, select Occupation from Social, and then select Add. Enter the employment and employment status in the corresponding fields. Enter phone number and restrictions in the corresponding fields. Select occupation from the list in the Occupation field. To clear and select another occupation, select Cl button and select occupation from the list. To select the retirement date, select the Retired field. Save and close the template. Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields. The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template If the patient is married, enter the spouse name. Update Past Medical History

You can update the past medical history for a patient in the Edit Past Medical History template. Open NextGen® Enterprise EHR. Open the Histories template. In the Medical/Surgical/Interim panel, select the medical or surgical record from the grid that you want to update, and select Edit. The Edit Past Medical History template opens. Edit Past Medical History Note: You can select an entry in the Past Medical History grid and can select Edit to open the Edit Past Medical History template. Update the necessary details.
