Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links enable you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission check box at the top of the template

Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. Adult Social History The Social History Review section enables you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History check box in the Social History section of the Practice Configuration practice template to set the template as default. PRAPARE® Assessment PRAPARE® Assessment You can document information related to a patient’s social determinants of health from the Protocol for Responding to and Assessing Patient's Assets, Risks, and Experiences (PRAPARE®) form on the PRAPARE® Assessment Tool template. Providers and health centers can analyze and act on the collected data as part of the national effort to better understand the socio-economic drivers that lead to poor health outcomes and increased clinical care cost for the patients. The PRAPARE® form presents questions specific to the following areas: Personal characteristics Family and home Money and resources Social and emotional health Practice defined information Based on a patient’s response to the questions, the system calculates and displays the score and interpretation (risk level) associated with the social determinants of health data of the patient. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Select Save & Close. Categories in Social History You can use different sections in the Social history to document the history of a patient. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template.
