PRAPARE® Assessment
You can document information related to a patient’s social determinants of health from the Protocol for Responding to and Assessing Patient's Assets, Risks, and Experiences (PRAPARE®) form on the PRAPARE® Assessment Tool template.
Providers and health centers can analyze and act on the collected data as part of the national effort to better understand the socio-economic drivers that lead to poor health outcomes and increased clinical care cost for the patients.
The PRAPARE® form presents questions specific to the following areas:
- Personal characteristics
- Family and home
- Money and resources
- Social and emotional health
- Practice defined information
Based on a patient’s response to the questions, the system calculates and displays the score and interpretation (risk level) associated with the social determinants of health data of the patient.
On the PRAPARE® Assessment Tool template, you can:
- View the historical data of PRAPARE®
- Document a patient's social determinants of health and calculate the score
- Include necessary services and submit the PRAPARE® assessment for review
- Generate a PRAPARE® document
