You can document a patient's social determinants in the PRAPARE® assessment tool.
If the patient declines to perform the assessment, you can document that as well. When you select the Decline PRAPARE® check box, all documented details, except the automatically filled information, will be cleared. If you continue to document details after declining, the Decline PRAPARE® check box will be cleared.
- Open the PRAPARE® Assessment Tool template.
- To change the date, select in the Performed Date field.
By default, the encounter date appears in this field. You can only enter the current or a past encounter date.
- To view the PRAPARE® details entered in the previous encounters, select the PRAPARE® Flowsheet link.
- To edit the patient's personal information, select Personal Information.
By default, the patient's basic demographic details are filled from the Modify Patient Information template.
- Document the patient's personal characteristics, family and home information, and money and resources information.
- For Federal Poverty Level (%FPL), select a value. If you have configured %Pov in the UDS panel of the Modify Patient Information template, then you must select the same value.
- Document the patient's social and emotional health and answer any additional questions that the patient responds to.
- Document answers for practice defined questions.
Note: You must enable the practice defined questions for the PRAPARE® form on the
Practice Configuration template. To overwrite existing practice-defined questions and responses with the recently updated questions and responses, select

- Select Calculate, then Save, and then Save & Close.
By default, NextGen® Enterprise EHR automatically fills the Total Score and Interpretation fields based on the configuration provided on the PRAPARE® Configuration template.
