Document the Social Determinants of Health Information

You can document the social determinants of health information of a patient.

1. Open NextGen® Enterprise EHR.
2. On the Intake template, select any specialty and Visit type as Office Visit.
3. To open the Social Determinants of Health template, do one of the following:
   • In the Histories template, on the Social panel, select Social Determinants of Health.
   • On the History Summary panel on Intake, go to Social, and select Social Determinants of Health.
   The Social Determinants of Health template opens.

   

4. Document the patient's response to each question.
5. If the patient does not want to answer the questions in a specific section, select the Declines to specify check box for that section.
6. Select Save to Grid or OK to upload the sections that calculate a score to the Screening Tool grid.
   Note: The applicable sections are displayed in the grid when you select OK.
7. If the patient changes a response to a specific question, simply document the updated response and select the Save to Grid or OK.
   The grid is updated for that section.
