Histories Template

Patient history information is documented on the Histories template for a new patient, and reviewed and updated for an existing patient. Histories Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Histories Template

You can add, update, delete, and review a patient's history summary details. The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. You can also add skin cancer details. Documenting the Patient History Summary You can add, update, delete, and review a patient's history summary details from the History Summary panel. Patient's Problem list Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Generate the Intake Note You can generate the intake note for a patient using Generate Intake Note in the Intake and Histories template. Document the OB Histories Template

To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History Panel You can document pregnancy history information in the Pregnancy History panel. Document the Gravidity/Parity Panel You can quickly view the patient's gravidity and parity and can only document multiple births information in the Gravidity/Parity panel. Document the Since Last Menstrual Period Panel You can document last menstrual period information in the Since Last Menstrual Period panel. Cardiology Histories Workflow

You can document the cardiology histories in the Histories - Cardiology template. The template uses standard framework functionality and is used by the nurse/technician to document a patient's history. You can document the following in the Histories – Cardiology template. Update Past Medical History You can update the past medical history for a patient in the Edit Past Medical History template. Documenting the Patient History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Patient's Medical History

The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. If no medical or surgical history exists, you can select the No relevant past medical/surgical history check box. You can filter the past history records by selecting the required check boxes. By default, the Show All check box is selected. Medical/Surgical/Interim panel On the Past Medical History template, the medical conditions and the surgical procedures associated with the current specialty appear alongside a check box in the respective panels. The Medical and Surgical panels include individual and grouped conditions or procedures. The individual conditions or procedures includes only a check box. The grouped conditions or procedure include [+] sign adjacent to the condition which indicates that the condition, or procedure includes sub-items. The conditions or surgical procedures, or sub-items with an asterisk (*) indicate that the condition or procedure, or sub-item is not a
