Overview of the Histories Template

You can add, update, delete, and review a patient's history summary details. You can document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history on the Histories template. Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. Patient's Social History You can document a patient's social history. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Patient's Diagnostic Tests and Results

You can document a patient's diagnostic tests and the associated results. Diagnostic Studies You can filter diagnostic studies using All or Specialty. The All option enables you to view the diagnostic studies relevant to all the specialties. The Specialty option enables you to view the diagnostic studies relevant to a specific specialty. Update a Historical Diagnostic Study When you update a historical diagnostics study on the Diagnostic Study - Orders template, the system automatically updates the corresponding studies reviewed record in the Reason for Visit panel. Delete a Historical Diagnostic Study You can delete a diagnostic study from the Diagnostic Study - Orders template grid. Document the Medical/Surgical/Interim Panel

You can filter the past history records in the grid by selecting the Medical History/Surgical History/Interim History check boxes. By default, Show All check box is selected. Open the Histories tab. Open the Medical/Surgical/Interim panel. Select Add to open the Past Medical History template. Select the check box corresponding to the correct entry. Note: Items in blue are categories rather than individual selections. Selecting the blue name, or selecting the check box opens a list of choices. The Add Pregnancy Detail button in the Medical panel opens the Pregnancy Outcomes template. Select the Add to Grid button. Past Medical History Expand Surgical to enter past procedures/operations. Surgical panel on the Past Medical History template Select Save & Close. A new past medical history record displays in the panel on the Histories template. Note: If information is entered in the Medical/Surgical/Interim grid during prior visits or by other specialties, that information also displays. Doc

Histories Template

Patient history information is documented on the Histories template for a new patient, and reviewed and updated for an existing patient. Histories Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Patient's Medical History

The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. If no medical or surgical history exists, you can select the No relevant past medical/surgical history check box. You can filter the past history records by selecting the required check boxes. By default, the Show All check box is selected. Medical/Surgical/Interim panel On the Past Medical History template, the medical conditions and the surgical procedures associated with the current specialty appear alongside a check box in the respective panels. The Medical and Surgical panels include individual and grouped conditions or procedures. The individual conditions or procedures includes only a check box. The grouped conditions or procedure include [+] sign adjacent to the condition which indicates that the condition, or procedure includes sub-items. Document the OB Histories Template

To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History Panel You can document pregnancy history information in the Pregnancy History panel.
