Open Problem List

You can access the problem list to document and view a patient's problem history. Open NextGen® Enterprise EHR. Select a patient record. To access the Problem List panel, do one of the following: In the Histories template, expand the Problem List panel. In the Home Page template, expand the Problem List panel. Note: You can use Problems in the quick button panel to access the Problems module. Add Problems in the Problem List You can add a problem to the problem list of a patient. Patient's Problem List

You can manage a patient's problems. You can perform the following actions in the Problem List panel. Record, modify, and retrieve a patient's clinical problem list electronically to meet Quality Reporting requirements Document the patient's chronic conditions and ongoing clinical problems View problems from past encounters Add problem details Problem List You can also indicate if a patient's documented problem is completed and reviewed, and if there is any active problems. Problem List

The Problem List tab enables you to view the details of the active, inactive, resolved, and unresolved problems of a patient and update them as needed. You can record and update a patient’s problems using the Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT®) concept. SNOMED-CT® is a standard vocabulary used to express clinical terms. SNOMED-CT® is comprised of descriptive terms or synonyms related to a clinical concept and provides links between different concepts. The problem list is demographic and is not associated with an encounter. After you record a problem if you need to use it as a diagnosis for billing, you must add the problem to the billing ICD list. Problem List

The top half of the Problem List tab displays all the problems in a patient's chart. You can modify certain details associated with each problem. The patient's problems appear based on their statuses in a tree-view format. The problems are categorized as active, inactive, recurrence, relapse, remission and resolved. You can expand or collapse the tree view using the and buttons. Problem List Problem List Toolbar The problem list toolbar displays options to modify the list of problems. Problem List Columns Select the problem list columns to display the details of the problems. Filtering Problems in the Problem List Set a criteria for the problems displayed in the problem list. Ungroup and Group the Problems in the Problem List You can group problems in the problem list according to the preferred description, and also ungroup them when required. Add a Problem to a Patient Chart

Open the Problems Module. On the Problem List tab, select Add Problem. The Problem Search window opens. To search for the patient's problem, do the following: In the search field, enter the problem description in full or in parts. Select Search. The problem search results appear. Problem search results Select a problem, and then select Select to add the problem to the patient's encounter. Close the Problem Search window. The selected problem is added to the Unresolved group in the problem list. Note: By default, the Problem Status of the new problem is set to Active. Note: For a new problem created from a template trigger, the Problem Status is set to Active and Verification Status is set to Confirmed by default. Modify the problem list data entry fields. To add the problem to patient's record, select Save.
