Problem List Data Entry Area

The problem list data entry area occupies the bottom half of the Problem List tab. The problem list data entry area displays the data entry options to add, modify, track, and resolve patients' problems. Problem List The problem list data entry area in the Problem List tab has the following three components: Data entry area toolbar Tracking toolbar Data entry fields Problem List Data Entry Area Toolbar The problem list data entry area toolbar provides functions to modify the selected problems. Managing Problems and ICD Codes

The Problem List and the Billing ICD List enable you to add, modify, resolve, and delete patient diagnosis, problem information, and Billing ICD entries. Add a Problem to a Patient Chart You can add a problem to the patient chart that can later be modified or updated. Viewing or Adding Synonyms in the Problem List Modify a Problem in the Problem List You can update the patient's problem for an unlocked encounter. Duplicate a Problem You can recur patient's problem for an unlocked encounter. Categorize a Problem Delete a Problem in the Problem List You can delete a problem from the patient's Problem List if it is entered by mistake or no longer relevant. Problem List Toolbar

The problem list toolbar is located above the problem list. Problem List The problem list toolbar provides the following options: Refresh: Updates all information that you entered and accepted in the Problem List tab. Preferences: Displays a list of settings for: The data display area for column display. Grouping problems. Enabling selections for SNOMED searches. Deleted Items: Displays the Deleted Items window which lists all deleted problems from the Problem List tab. Status Selection: Enables you to specify whether you want the data display area to only show the problems with Active status. Show My Tracked Problems Only: Displays only those problems that you added to your tracked problems list. Show Chronic Problems Only: Displays only those problems you specified as Chronic. Ungroup and Group the Problems in the Problem List

By default, problems are grouped according to the preferred description, and the SNOMED® fully specified name. To ungroup the problems in the problem list, select Preferences in the problem list toolbar, and select Ungroup. The problems merge into Active, Inactive, Recurrence, Relapse, Remission, or Resolved groups. Problem List To group the problems in the problem list, select Preferences and select Regroup. The problems in the problem list appear in a tree-view structure. Do any of the following: To expand the tree-view structure for all problems, select Preferences and select Expand All. Filtering Problems in the Problem List

You can set criteria on the problem list toolbar to specify the problems you want to view in the problems list. You can view active, tracked, and chronic problems. Open the Problems Module.. In the problem list toolbar, select the Status Selection list, and do one of the following: To view all active and resolved problems, select Show All Statuses. To view only the active problems, select Show Active Problems Only. To view the problems that you added to your tracked problems list, select the Show My Tracked Problems Only check box. To view the problems that are specified as chronic, select the Show Chronic Problems Only check box. Problem List

The Problem List tab enables you to view the details of the active, inactive, resolved, and unresolved problems of a patient and update them as needed. You can record and update a patient’s problems using the Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT®) concept. SNOMED-CT® is a standard vocabulary used to express clinical terms. SNOMED-CT® is comprised of descriptive terms or synonyms related to a clinical concept and provides links between different concepts. The problem list is demographic and is not associated with an encounter. After you record a problem if you need to use it as a diagnosis for billing, you must add the problem to the billing ICD list.
