Access the Add or Update Assessment Template

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click Add/Update. The Add or Update Assessment template opens. Open the Assessments Tab

There are several options for accessing the Add or Update Assessments template: On the SOAP template, in the Assessment / Plan panel, select Add/Update. On the Intake or SOAP templates: Select the My Plan link to open the template set. My Plan Select the Assessments tab. Delete an Existing Assessment

CAUTION: Removing an item from the Today's Assessment grid does not resolve the diagnosis. The Add or Update Assessments template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates as the working assessment. If you remove one of multiple available assessments on the Add or Update Assessments template, you must select a new working assessment that carries over to the other templates that share the same working assessment. Using the Assessments Tab

The Assessments tab in the My Plan template set houses the Add or Update Assessment template. The grids on the Assessments tab can be filtered. For example, in the Billing Diagnosis History section, selecting the Show Chronic only check box displays only those diagnoses that are not acute. Selecting the option to Add Assessments on 1 click speeds up the documentation process by including the diagnosis description and code for Today's Assessment in the grid and Problems module without having to click Add/Update. The Medications or Immunizations links can be used at any time during the encounter to access the associated module. Access the Add or Update Assessment Template

Select the Add/Update button below the Today’s Assessment grid on the MICS MedAdmin OPH template. The Add or Update Assessment template opens. Diagnoses already attached to treatments for this patient appear in the Billing Diagnosis History grid in the upper left. Add or Update Assessment Template

The Add or Update Assessment template that launches from the Behavioral Health templates differs from the core specialties. In this version, the template is updated to reflect the standards put forth in DSM-5 by incorporating the Severity and Specifiers into the data collected into a grid, among these other changes: The Site and Axis fields are hidden when the specialty is set to Behavioral Health. The Severity field launches a picklist with the following selections: Mild Moderate Severe Persistent Extreme The grid includes the following headers: # - the priority number of diagnoses entered for Today's Assessment.
