Enter the Medication Allergy Details

  1. Once you selected an Allergen and Allergy Type, select a Location and Provider.
  2. Select the Intolerance check box if the patient is intolerant to the medication.

  3. To specify the severity of the allergy, select a Severity from the list.
  4. To specify that the allergy you are adding is recorded elsewhere, select the Recorded Elsewhere check box.
    The Source field is available to enter a source.
  5. To enter the date the allergy or its symptoms began, click the Onset/Sym Search button. A calendar opens to select an onset date.
  6. To enter the date the allergy was resolved, click the Resolved Search button. A calendar opens to select a resolved date.
  7. To specify the type of reaction the patient has due to the medication allergy, click the Reaction arrow and select a reaction type from the list.
  8. Verify the Allergy Type field specified in the Allergy Selection window.
  9. Enter additional comments in the Allergy Comments field.
  10. To save the medication allergy, click Add.
  11. Click Clear to reset the data fields to their default values to add another allergy.
