Enter Patient Allergy Data
The Allergies panel on the BH - Nurse Note template is used to add or review allergy information.
- If no allergies are reported by the client, select the No known allergies check box.
- If the client indicates a new allergy has presented since the last visit, click Add and do the following:
- Select the allergen from the list.
- Enter a Reaction from the list that automatically launches or click on the top blank row to type a reaction.
- Repeat for any other new allergies.
- Click Save & Close.
- Select the Reviewed, updated option.
Note: The severity of the reaction cannot be added on the Allergies panel. Go to the Medication Allergies module to add the severity of the reaction, where multiple reactions and severities can be selected. After the severity is added in the Medication Allergies module and updated, the severity appears in the Allergies panel grid under the Reaction (Severity) column.
- If a conversation with the patient reveals no new information for existing allergies, select Reviewed, no change.
