Add an Allergy Reaction and Severity

You can document an allergy reaction and severity for the allergy. Reaction-severity details appear wherever allergies display. Although allergy reactions and severities are SNOMED® codified, you can create user-defined reactions but not user-defined severities. Contact your system administrator for more information. From the Medication Allergies module, in the Reactions section, select Add. The Add Reaction (Severity) window opens. Reactions marked as common list first. Begin typing the name of a reaction or scroll through the list of reactions, and then select a reaction. Unknown is located at the top of the Common Reactions list. Specify a severity of the reaction. Select Add. The allergy reaction and severity appear in the Reactions section. You can add up to five allergy reaction and severity. Add a Criticality

You can document criticality values per allergy in the Criticality field. You can select Low, High, or Unable to Assess from the list. The selected criticality value displays in the Description (Criticality) column in the Medication Allergies grid. Open the Medication Allergies module for the selected patient. Select the High, Low, or Unable to Assess criticality value. The Criticality value appears in the grid. Note: If you select Unable to Assess the criticality value does not appear in the grid. Open NextGen® Enterprise EHR. Open the Medication Allergies module for the selected patient. If the patient has any existing allergies, select Clear. The values in all the fields are cleared. Note: Clearing the fields prevents you from editing an existing allergy. To search and filter an allergy, do the following: Select the search icon next to Allergy. The NextGen - Allergy Selection window opens. Allergy Selection Select one of the following filters from the Filter list: Contains Searches for allergies that contain with the search criteria. Ends With Searches for allergies that end with the search criteria. Ingredient Searches from the list of drug ingredients, such as Iodine or Bentonite. Medication Name searches from the list of medication allergies, such as cefazolin with codeine. Allergen Group Searches from the list of allergens, such as lithium derivatives or ester type anesthetics. Update Medication Allergy

You can update a medication allergy in the patient's chart. If the encounter is locked, you do not have to unlock the encounter or create a duplicate allergy entry. If the encounter is locked, you can update the Intolerance, Criticality, Severity, Reactions, Add Additional Reaction, Onset of Symptoms Date, Resolved Date, and Allergy Comments fields. The following buttons are enabled: Update, Interactions, and Reconcile. Open the Medication Allergies module. Select an allergy. Enter the changes to the available information. Select Update. View Patient's Medication Allergies

Before you begin You must ensure that you have the View access right enabled in MODULES > Medical Records > Medications Allergies in System Administrator. For more information on user access rights, go to NextGen Healthcare Success Community, and download the latest System Administrator Guide forNextGen®Enterprise. You can view a quick list of the patient's active medication allergies to identify potential adverse drug reactions. You can also open the Medication Allergies module to add or update the patient's allergies. A total count of the patient's active allergies appears on the Allergies button on the Quick Buttons panel. Note: The icon indicates that the patient account does not have any active allergies records and the No Known Allergies check box in Medication Allergies is not selected. Delete Medication Allergy

Before you begin Ensure the encounter that you select to open Medication Allergies module is unlocked. If you need to delete a medication allergy assigned to a locked encounter, you must create either a custom or a new encounter and add a new allergy with the modifications to an existing allergy. Open NextGen® Enterprise EHR. Open the Medication Allergies module for the selected patient. Select the allergy you want to delete from the module grid and select Delete. The Delete Record window opens displaying the confirmation message. Do one of the following: You can select Delete and mark item "Inactive" if you want to make the allergy inactive. You can select Delete and mark item "Entered in error" for an allergy that is incorrectly added by provider. Select Yes to delete the allergy. Select No or close the window to cancel the deletion.
