Document Medication Information

On the BH - Nursing Note template, navigate to the Medication Information panel and answer the following questions with No, Yes, or N/A (Not Applicable) and enter Comments as needed: Takes medications as prescribed Changes in medical status Reviewed medication name(s), dosage, purpose, and frequency Were medications delivered today - If yes, for what duration Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open a template on which medication side effects can be documented. Next to Other medications, select from the following options: None Herbal Over the counter Other - and type specifics in the associated field Click the Office Procedures link to launch the My Plan tabbed template set on the Office Procedures tab and document medications administered today. Enter Allergy Data

On the BH - Psychopharm template, expand the Medication Information panel. In the Allergies section, select No known allergies, Allergies added today, or Reviewed, no change. Click Add to add new allergies for the patient. The Adult Allergies or Pediatric Allergies template appears. Select the allergen reported by the patient. The Allergy Reaction list appears. Enter a Reaction from the list that automatically launches or click on the top blank row to type a reaction. Click Save & Close to close the template and enter the data into the grid. Select the Reviewed, updated option. Note: The severity of the reaction cannot be added on the Allergies panel. Go to the Medication Allergies module to add the severity of the reaction, where multiple reactions and severities can be selected. After the severity is added in the Medication Allergies module and updated, the severity appears in the Allergies panel grid under the Reaction (Severity) column. Enter Allergy Data on Psychiatric Evaluation

On the BH - Psychiatric Evaluation template, expand the Medication Information panel. In the Allergies section, click the Comments link to include relevant details. Select No known allergies, Reviewed updated, or Reviewed, no change. Click Add or Update to enter data on the Adult Allergies or Pediatrics Allergies template. Note: The severity of the reaction cannot be added on the Allergies panel. Go to the Medication Allergies module to add the severity of the reaction, where multiple reactions and severities can be selected. After the severity is added in the Medication Allergies module and updated, the severity appears in the Allergies panel grid under the Reaction (Severity) column. Adding and Reviewing Allergies

You can add and review the allergy details associated with a patient in the Allergies panel. You can add an allergy and its reaction on the Allergies panel. You must add the severity level on the Medication Allergies module which then displays the severity level in the Reaction (Severity) column in the Allergies panel. On the Medication Allergies module, you can select multiple reactions and the respective severities for each allergy. Allergies You can select the No known allergies checkbox to indicate that the patient does not have any known medication allergies. You can select the Reviewed, Updated checkbox to indicate that the allergy details are reviewed and updated during the encounter You can select the Reviewed, no change checkbox to indicate that the patient does not have new details to add. Allergies

You can manage patient allergy details of a patient. You can add and review the allergy details of a patient in the Allergies panel. The Allergies panel allows you to add and review the allergy information associated with a patient. If there are no known allergies for a patient, you can select No known allergies. Allergies Panel You can only add the allergy and its reaction in the Allergies panel. The severity level must be added from the Medication Allergies module. For an allergy record, you can select multiple reactions and their respective severity levels from the Medication Allergies module. You can view the intolerance details for the added allergies in the Allergies panel if the Intolerance checkbox is selected on the Medication Allergies module. The Intolerance checkbox indicates that the patient cannot tolerate the side effects of the medication. Edit an Allergy Reaction and Severity

From the Medication Allergies module > Reactions section, select a reaction, and then select Edit. The Edit Reaction Severities window opens. Select a reaction, and then select a different severity. Note: Alert messages do not appear when you are updating the reaction severity. If the provider does not select a new severity value, the patient’s chart retains its current value. If necessary, select another reaction, and change its severity. An alert icon (asterisk) displays next to the allergy that contains the non-supported severity or hidden values. If applicable, select a new severity value for the unsupported severity. Select Cancel if you do not want to update to the new supported severity values. Note: The existing unsupported severity values are retained in the patient’s chart. However, they are mapped to the supported values while exporting to the C-CDA.
