Dynamic HPI Config Practice Template

The Dynamic HPI Config practice template enables you to configure elements for the dynamic History of Present Illness (HPI) templates. Each dynamic HPI template is linked to a specific reason for visit. The dynamic HPI templates that are packaged with NextGen® Adaptive Content Engine can be modified to meet the specific needs of a practice. If there is a need for a custom reason for visit, the practice can create a custom template. Data sets for labs, diagnostics, and procedures can be configured in the Section and Finding sections. New sections such as family history, lab data, and diagnostic data can be configured for a reason for visit. The new sections appear in the dynamic HPI template in the left pane under the Reason field in run time. The Contradiction setup link enables you to create guidelines that prevent users from selecting clinically contradictory findings. Dynamic HPI Contradictions Config Practice Template

The Dynamic HPI Contradictions Config template is designed to prevent you from selecting clinically contradictory terms in the same section or sub-section of a dynamic HPI. For example, you would not want to document a Quality of Mass as tender and non-tender in the same patient encounter. By making these values mutually exclusive, the system prevents the user from accidentally selecting the opposing terminology simultaneously. Any practice can use the Dynamic HPI Contradictions Config template to manually create new, mutually exclusive selections, and modify or remove existing pairs. Contradictions can also be matched automatically by selecting the Automatically match similar contradictions link. The practice template contains two grids: Findings and contradictions: Lists content items and their contradictions within a specific section or sub-section. Data Sets Setup Practice Template

The Data Sets Setup practice template stores codes and text descriptions for specific tests. The test name must match the Test Name value from the Test Dates data set on the practice template. The DHPI Test Codes data set on Data Sets Setup enables you to apply synonyms for test results on the NextGen® Adaptive Content Engine templates. The synonyms represent descriptions or codes that map to a single test or procedure. The synonyms are used to reference the test in the Protocol data set or the Test Name in the Test Date data set. The DHPI Test Code data set grid includes the following columns: Test name - used by the practice to identify an order for a diagnostic, patient procedure, or lab Codes - the code used by the reference lab or outside study/procedure location to identify the test at their location for ordering or billing purposes Description - the description for the test used by the reference lab or outside study/procedure location Add a New DHPI Test Code to a Data Set Test

Create or Edit Sentences for a Dynamic HPI

You can configure the concatenation of the dynamic HPI findings. Navigate to the Findings section of the Dynamic HPI Config practice template. Click Edit/Create new sentence. The Dynamic HPI Sentences Config template opens. Dynamic HPI Sentences Config Click the Pre Text field to enter the text for a new sentence. The character limit is one hundred (100) Click the Information icon to view the functions to enter in the Pre text field. Sentence Functions Click Add Function to select a value from the HPI Functions window. HPI Functions If you have the Pre Text as "[FNAME] [LNAME] is a [AGE] [GENDER]", the dynamic HPI template displays the patient's name, age, and gender as 'John Smith is a 45 year old male' Example for sentence structure In the Include section, select Finding to include a finding in a sentence. Dynamic HPI Findings for a Reason

The Dynamic HPI Config practice template enables you to build different findings based on the associated sections for each reason of visit in the Findings section. Findings The Selection Type and Parameter field determine how the findings are presented on the dynamic HPI. The documented value in the Finding Type field determines how the sentence appears in the dynamic HPI template: The mutually exclusive item displays the selected finding as a separate sentence within that section. The multichoice single item enables the user to select multiple findings to be concatenated into a single sentence structure. The multichoice multiple item enables the user to select multiple findings so that each finding has its own sentence structure.
