Document the Dental Caries Risk Level and Sealants Details

You can document the dental caries risk level and sealant details of a patient using the Dental Caries Risk/Sealants template. The details documented on the Dental Caries Risk/Sealants template appear in the patient plan document. You can document the dental care details for adults on the Dental Caries Risk/Sealants template. For a pediatric patient, you can document the dental care details on both Histories and Dental Caries Risk/Sealants template.

  1. To open the Templates module on a patient encounter, do one of the following:
    • On the Categories or Encounter History tab, select Open Template.
    • On Top Toolbar, select Templates .
    • On the main menu, select File > New > Template.
    The Select Template window opens.
  2. On the Medical Records tab, select All.
  3. Select Home Page - Dental and click OK.
    The Home Page - Dental template opens.
  4. Set the Specialty as Dental and any Visit Type.
  5. On the Home Page or Histories template, click Dental Care.
    The Dental Caries Risk/Sealants template opens.

  6. In Dental Care, do one of the following:
    • Click the Last visit field and select the patient's last visit date.
    • If there are no dental problems, select No concerns
    • If there are dental concerns, do the following:
      • In Concerns, type the details of the concern.
      • Select either No or Yes for Caries, Erosion, Disease, and Topical fluoride applied.
      • If you have selected Yes for Topical fluoride applied, type the name of the provider in the By whom field.
      • Select the date on which the patient received the treatment in Date received.
  7. In Caries Risk, do the following:
    • Click the Risk level field and select a risk level. If the desired option is not available in the list, click the top blank row to enter the risk level of a patient.
    The risk level that you have entered has been added to the list and is available for future encounters.
    • Select the date on which the risk level has been evaluated by clicking the Examined on field.
  8. In Sealants, do the following:
    • Click the Tooth applied field and select the tooth on which the sealant has been applied. If the desired option is not available in the list, click the top blank row and enter the type of tooth on which the sealant has been applied.
    The type of tooth that you have entered has been added to the list and is available for future encounters.
    • Click the Sealant applied field and select a Sealant from the list. If the desired option is not available, click the top blank row and type the sealant applied.
    The type of sealant that you have entered has been added to the list and is available for future encounters.
    • Select the date on which the sealant has been applied to the tooth in the Date applied field.
    The current user name is automatically filled in the Documented by field.
  9. Click Add/Edit.
    The grid at the bottom of the Dental Caries Risk/Sealants template has been automatically filled with the patient's record. You can edit the record, if needed.
  10. Click Save & Close.

Open the Template module

To open the Templates module on a patient encounter, do one of the following:
  • On the Categories or Encounter History tab, select Open Template.
  • On Top Toolbar, select Templates .
  • On the main menu, select File > New > Template.
The Select Template window opens.
