Document the Patient's Status

  1. Open the Histories template.
  2. In the Social panel, select Statuses, and then click Add/Edit.
    The Social History - Status template opens.

  3. In the Demographics section, document the patient's race, ethnicity, and preferred language, the language they speak at home, and their dominant hand.
  4. In the Employment section, do the following:
    1. Click Add, and on the Employment/Occupation – [New Record] template, enter the patient's occupation, employer information, employment status, restrictions, and other details.
    2. Click Save, and then close the template.
    3. Select the occupational hazards or enter the hazards manually. You can enter up to two hazards.
  5. In the Radiation Exposure section, document the exposure date, radiation type, and the radiation dose and unit.
    By default, the encounter date appears as the exposure date. To select another date, click Cl.
  6. To view the radiation exposure history, click the Exposure History link.
  7. In the Military section, indicate whether the patient has military experience, and do the following:
    1. If you select Yes, enter the current status, military branch, years served, and type of discharge.
    2. Select the appropriate option for Noise exposure, Biohazard exposure, and Stationed Overseas.
  8. In the Education section, document the patient's education, degree, and the country the degree was obtained from, and do the following:
    1. To document additional educational details, click the Details link.
    2. On the Education Detail template, enter the school name, school type, and grade.
    3. Select the appropriate option for Learning disability, Special needs, and Gifted Program.
    4. If the patient has repeated any grades, select Yes, and enter the grade and reason.
    5. Indicate whether the patient was suspended or expelled from school, and enter the details.
    6. Click Save & Close.
  9. Enter the details in the Marital Status/Family/Social Support section.
  10. Save and close the template.
