Document SWYC Developmental Milestone by Family Member

The Survey of Well-being of Young Children (SWYC) is a comprehensive screening instrument for children below 5 years of age. You can document development milestones for the family members or caretakers of the child.

Before you begin

To access the SWYC Milestones templates, you need to select the Use SWYC development screening check box on the Practice Configuration template. If the Use SWYC development screening check box is not selected, you can open the SWYC Milestone Family View template using Templates on the Top Toolbar. 1. Open the Pediatric SOAP template.
2. In the Screening panel, click the Child Development History button.
   The SWYC Milestones Full View template opens.
3. Click the Child Development Family View tab.
   The SWYC Milestones Family View template opens.
4. On the menu bar, click View, and select Patient Mode.
5. Click OK on the confirmation message to proceed.
   The SWYC Milestones Family View template opens in Patient Mode.

   

6. On the Survey of Well-being of Young Children© panel, click the Details link on the age group appropriate to the child's current age.
   The corresponding SWYC Child Development Milestones template opens.

   Note: If the development milestone template is not appropriate to the child's current age, the Age Out of Range alert message appears. Click OK to continue the use of SWYC Child Development Milestones template for children with developmental delays.
7. Select the appropriate option for each milestone.
   Note: The family member or caretaker must answer all the developmental milestone questions to view the Save & Close button on the template. After development milestones are documented by a family member or caretaker of the child, you must change the template view from Patient Mode back to Full Screen mode.
8. To change the template view from Patient Mode to Full Screen mode, do the following:
   1. After development milestones are documented, press Esc on your keyboard.
   2. Click Yes on the confirmation message to exit the patient mode.
   3. Enter your application password and click Logon.
   The SWYC Milestones Family View template opens in the Full Screen mode.

   The documented milestones and total score appear in the Screening Tool Results grid on the SWYC Milestones Full View template.
