Stage Cancer from the Onc Treatment Summary Template
The Onc Treatment Summary template provides the most comprehensive method of entering cancer staging and related data, including cancer medications, treatment notes and other data. For Cancer Registry Report, you must provide data on the following: Dx Date (Diagnosis Date) Cancer Type/Site Histology Behavior Laterality Body Site Diagnostic Confirmation Stage a Cancer Prior to staging cancer, you should enter the cancer diagnosis for the patient. If you do not, you can select a diagnosis during the procedure. Cancer Registry Reporting
You can document the diagnosis, evaluation, and treatment of active cancer for cancer registry reporting purposes and generate the Cancer Registry Report. Cancer Registry Reporting During a patient encounter, the cancer registry functions enable you to document the diagnosis, evaluation, and treatment of active cancer for cancer registry reporting purposes. Record Patient Demographic Data
Access the Patient Demographics template and scroll to the Patient Information panel. Patient Information Enter the following information, if not already entered: Patient Name (first, middle & last) Ethnicity Marital Status Race Country of Birth State of Birth Note: If required information is not known, enter Unknown. Generate the Cancer Registry File
When all required demographic and cancer staging data has been entered, scroll to the bottom of the Onc Treatment Summary template and click the Required Fields Check button. Diagnostic History The Missing Data template appears and displays a list of outstanding required fields that must be completed to generate a Cancer Registry report. Missing Data Record the list of missing data and then click Close. Enter any missing data. Close all open templates and modules. Right-click the encounter that contains the cancer diagnoses and select Lock Encounter. The NextGen EHR Security Lock Encounter window appears. Lock Encounter Enter your User Name and Password. Click Lock. The system generates the Cancer Registry Report and saves the file to the export folder designated in the Rosetta Cancer Registry File Export agent settings. Access the Onc Treatment Summary Template
Open a patient encounter and select a Specialty of Oncology and a Visit Type of Office visit. Note: Ensure the patient has an appropriate cancer diagnosis code in the Billing ICD list. The diagnosis code must match a valid CRR Report Code. Document the condition in the patient’s Problem List module. Select Cancer Treatment Summary from the navigation bar. The Onc Treatment Summary template appears. Enter Cancer Data
Access the Onc Treatment Summary template and scroll to the Notes panel. Click the Type field and select Progress from the list. Type a progress note in the data entry area and click Add. The progress note appears in the grid. Click the Type field and select Radiation Oncology from the list. Type a radiation oncology narrative in the data entry area and click Add. The narrative appears in the grid. Scroll to the Problem List panel, click Add or Edit, and enter the following information: Problem Description Onset Date onc treatment summary template prob list Return to the Onc Treatment Summary template and select Tobacco Cessation from the left Navigation menu. The Tobacco Cessation template appears. Click the Tobacco Usage link. The Social History – Tobacco template appears. Enter the patient’s smoking use information and click OK.
