Calculate and Submit the CPT Codes for Transition of Care Visits

You can calculate and submit the CPT codes for the transition of care visits.

1. Open NextGen® Enterprise EHR.
2. Select a patient and open an encounter.
3. Select Specialty as Family Practice and Visit Type as Transition into Care.
4. On the Intake template, click the Medications panel.
5. Click the Medications Reconciled check box.
6. Click the Finalize tab.
   The Finalize template opens.

   

7. Under the Today's Assessment panel, add an assessment, if an assessment has not been added.
8. Click the Evaluation and Management Coding panel.
   Notice that the Transition to Care Face to Face Visit check box is selected by default (even if it is not a face-to-face visit). This does not appear in any other place.
9. In Medical Decision Making, select a complexity level.
10. Click Calculate Code.
    The CPT code for the transitioning into care visit displays in the first Visit Code field.

    

11. Click Submit Code.
    The charges have been submitted to the Superbill.
