Calculate and Submit the CPT Codes for Transition of Care Visits

You can calculate and submit the CPT codes for the transition of care visits. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Select Specialty as Family Practice and Visit Type as Transition into Care. On the Intake template, click the Medications panel. Click the Medications Reconciled check box. Click the Finalize tab. The Finalize template opens. Evaluation and Management Coding panel on the Finalize template Under the Today's Assessment panel, add an assessment, if an assessment has not been added. Click the Evaluation and Management Coding panel. Notice that the Transition to Care Face to Face Visit check box is selected by default (even if it is not a face-to-face visit). This does not appear in any other place. In Medical Decision Making, select a complexity level. Click Calculate Code. The CPT code for the transitioning into care visit displays in the first Visit Code field. Click Submit Code. The charges have been submitted to the Superbill. Parent topic:

Enter the CPT Codes for Transition of Care Visits

Open NextGen® Enterprise EHR. Select a patient and open an encounter. On the Intake template, select an impacted Specialty (Family Practice/Internal Medicine, Obstetrics or Pediatrics), and select Visit Type as Transition into Care. Click the Finalize tab. The Finalize template opens. In the Evaluation and Management Coding panel, click the second Value Code field. A list of service categories opens. Evaluation and Management Coding Click Care Management. A list of visit codes and descriptions opens. Service Item Mstr Select the visit code that represents the complexity and timing of care. Click OK. Upon return to the Finalize template, the CPT code populates the second Visit Code field. Important: You must complete all the fields in the Transitioning Into Care or the Cm Care Transitions template before submitting the charges to the Superbill. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty impacted by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is impacted by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, click the Transitioning Into Care link. The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, click the Discharged field, and select the discharged date from the calendar. Click the Initial Communication field, and select the appropriate option from the list of communication types. Click the on field, and select the date of the initial communication from the calendar. Transitioning into Care Documentation

You can use the Transition Into Care visit type to document the medical and psychosocial problems that require moderate to high-complexity decision making related to different types of discharge. You can document the transition into care details on the Transitioning Into Care template along with other relevant details and verify if the appropriate CPT codes have been calculated and submitted to the Superbill. You can find the transitioning into care management services for the following specialties: Family Practice/Internal Medicine Pediatrics Obstetrics To bill for transitioning into care management, you can use one of the following two CPT codes: 99495 for transitional care management services with moderate medical decision complexity (face-to-face visit within 14 days of discharge). 99496 for transitional care management services with high medical decision complexity (face-to-face visit within 7 days of discharge). Document Transitioning into Care from the Care Management Specialty

You can document transitioning into care from the Care Management specialty. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select Specialty as Care Management and Visit Type as Care Management. The Cm Care Transitions template opens. Cm Care Transitions On the General panel, do one of the following: Click the Encounter Type field to open a list of transitional care encounter types and then select an encounter type from that list. Click the blue arrow to the right of the Encounter type field to open the interim history data list and then select a past encounter from the interim history data. Click the PCP field. If there is no PCP selected in the patient chart, the Modify Patient Information template opens, enabling you to select a PCP. Document General Information

After reviewing the available patient details on the Home Page template, you can document the general information of a patient in the General panel. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template. Click the General panel. Select the patient. If someone else is providing the patient details, select the relationship of the person with the patient in the Historian field. To capture the summary of care record information for a patient, click Transitioning into Care, and fill the information in the resulting window.
