Calculate and Submit the CPT Codes for Transition of Care Visits

You can calculate and submit the CPT codes for the transition of care visits. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Select Specialty as Family Practice and Visit Type as Transition into Care. On the Intake template, click the Medications panel. Click the Medications Reconciled check box. Click the Finalize tab. The Finalize template opens. Evaluation and Management Coding panel on the Finalize template Under the Today's Assessment panel, add an assessment, if an assessment has not been added. Click the Evaluation and Management Coding panel. Notice that the Transition to Care Face to Face Visit check box is selected by default (even if it is not a face-to-face visit). This does not appear in any other place. In Medical Decision Making, select a complexity level. Click Calculate Code. The CPT code for the transitioning into care visit displays in the first Visit Code field. Click Submit Code. The charges have been submitted to the Superbill. Parent topic:

Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty impacted by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is impacted by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, click the Transitioning Into Care link. The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, click the Discharged field, and select the discharged date from the calendar. Click the Initial Communication field, and select the appropriate option from the list of communication types. Click the on field, and select the date of the initial communication from the calendar. Enter Additional Visit Codes

You can enter two additional codes. The entered codes can be additional E&M codes, behavioral health codes, medicare preventive codes, or visit code service codes. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. In Evaluation and Management Coding, do the following: To enter a visit code service code, select the second or third Visit code field, and then select a code from the list. Note: You can update the items in the Visit Code Service Category list in File Maintenance under Service Categories. To enter an E&M, behavioral health, or medicare preventive code, select the corresponding links, and then select the code. Add Visit Code Modifiers You can add or replace visit code modifiers. Select a Prolonged Time Code You can select a prolonged time code to report time that surpasses the threshold for code 99205 or 99215 on the Finalize template. Enter Counseling Details

You can enter the counseling details for a patient and include them in the visit note. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. On Evaluation and Management Coding, select the Counseling Details link. The Counseling Details template opens. Counseling Details template In the Counseling/Educational Details section, do the following: Select the counseling type, method, and evaluation. The counselor's name and date appear. Enter the time spent in patient counseling. Enter additional counseling information. Enter the type of educational materials the patient received. To enter details about an interpreter, select the corresponding check box, and then enter their language, name, and relationship with the patient. Select a Prolonged Time Code

You can select a prolonged time code to report time that surpasses the threshold for code 99205 or 99215 on the Finalize template. Before you begin You must calculate or manually select the visit code 99205 or 99215 in the Evaluation and Management Coding panel. You can report a time of more than 74 minutes for a new patient for code 99205, or more than 54 minutes for an established patient for code 99215. Open NextGen® Enterprise EHR. Open the Finalize template. On the Evaluation and Management Coding panel, select the first blank Visit code field. The Visit Code Service Category Name window opens. Select Prolonged, and select the code for the prolonged service time. The prolonged service time code appears in the Visit code field. Select SNOMED CT Codes for Quality Reporting

If you do not bill from NextGen® Enterprise EHR but need to meet the visit-code criteria for Clinical Quality Measures (CQMs), you can use a SNOMED® CT code instead of a CPT® code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. On Evaluation and Management Coding, select the SNOMED Type (optional) link. The SNOMED Type template opens. Select a SNOMED® CT code. Select Save & Close. The NextGen® HQM application uses the selected code at the registry reporting time. Note: The code is not uploaded to the Procedures or Problems modules.
