Transitioning into Care Documentation
You can use the Transition Into Care visit type to document the medical and psychosocial problems that require moderate to high-complexity decision making related to different types of discharge.
You can document the transition into care details on the Transitioning Into Care template along with other relevant details and verify if the appropriate CPT codes have been calculated and submitted to the Superbill.
You can find the transitioning into care management services for the following specialties:
- Family Practice/Internal Medicine
- Pediatrics
- Obstetrics
To bill for transitioning into care management, you can use one of the following two CPT codes:
- 99495
- for transitional care management services with moderate medical decision complexity (face-to-face visit within 14 days of discharge).
- 99496
- for transitional care management services with high medical decision complexity (face-to-face visit within 7 days of discharge).
