Transitioning into Care Workflow

You can choose a workflow for documenting a transition of care management visit based on your starting point in the application. There are two workflows for documenting a transition of care management visit from which you can choose depending on your starting point in NextGen® Enterprise EHR. Transitioning into care from any of the impacted specialties (Family Practice/Internal Medicine, Pediatrics, and Obstetrics). Transitioning into care from the Care Management specialty. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty impacted by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is impacted by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, click the Transitioning Into Care link. The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, click the Discharged field, and select the discharged date from the calendar. Click the Initial Communication field, and select the appropriate option from the list of communication types. Click the on field, and select the date of the initial communication from the calendar. Enter the CPT Codes for Transition of Care Visits

Open NextGen® Enterprise EHR. Select a patient and open an encounter. On the Intake template, select an impacted Specialty (Family Practice/Internal Medicine, Obstetrics or Pediatrics), and select Visit Type as Transition into Care. Click the Finalize tab. The Finalize template opens. In the Evaluation and Management Coding panel, click the second Value Code field. A list of service categories opens. Evaluation and Management Coding Click Care Management. A list of visit codes and descriptions opens. Service Item Mstr Select the visit code that represents the complexity and timing of care. Click OK. Upon return to the Finalize template, the CPT code populates the second Visit Code field. Important: You must complete all the fields in the Transitioning Into Care or the Cm Care Transitions template before submitting the charges to the Superbill. If you do not, a message prompts you to enter the additional details. Click Submit Code. The charges are submitted to the Superbill. Parent topi

Calculate and Submit the CPT Codes for Transition of Care Visits

You can calculate and submit the CPT codes for the transition of care visits. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Select Specialty as Family Practice and Visit Type as Transition into Care. On the Intake template, click the Medications panel. Click the Medications Reconciled check box. Click the Finalize tab. The Finalize template opens. Evaluation and Management Coding panel on the Finalize template Under the Today's Assessment panel, add an assessment, if an assessment has not been added. Click the Evaluation and Management Coding panel. Notice that the Transition to Care Face to Face Visit check box is selected by default (even if it is not a face-to-face visit). This does not appear in any other place. In Medical Decision Making, select a complexity level. Click Calculate Code. The CPT code for the transitioning into care visit displays in the first Visit Code field. Click Submit Code. The charges have been submitted to the Superbill. Parent topic:

Document General Transition of Care Information for a Patient

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Click the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions Click the PCP field.The field populates with the Primary Care Practitioner established in the patient's demographics. If there is no PCP selected in the patient chart, the Modify Patient Information template opens, enabling you to select a PCP. Do one of the following: Click the Encounter Type field to launch a list of transitional care encounter types and then select an encounter type from that list.
