Calculate the E&M Code under 95 or 97 Coding Guidelines

Before you begin

You must configure the 95 or 97 Coding Guidelines selected on the Practice Configuration practice template. For more information on the 95 and 97 Coding Guidelines, go to the NextGen Healthcare Success Community and download the latest E&M Coding User Guide for NextGen® Adaptive Content Engine.

The '95 and '97 guidelines base the Evaluation and Management (E&M) code calculation on the selected Medical Decision Making (MDM) level and the documented history and physical exam elements or on the total visit time if more than 50 percent of the time was spent counseling or coordinating care.
  1. Open the Finalize - OPH template.
  2. If the patient was counseled, enter counseling and educational details and factors.
  3. Select the level of complexity associated with the encounter in the Medical Decision Making section.
  4. To calculate the code based on the total visit time, click the Total visit time (minutes) field, enter the total time in minutes, including counseling time, and click OK.

    You must enter a total visit time at least equal to the total counseling time; otherwise, an alert appears.

    The total time appears in the Total visit time (minutes) field.

    If the total counseling time is greater than 50 percent of the total visit time, the application selects the Counseled greater than 50 percent of time and documented content check box.

    If the total counseling time is less than 50 percent of the total visit time, an alert appears, and the Evaluation and Management Coding panel displays a message that the E&M code cannot be calculated based on time. You must select an MDM level to calculate the code.

  5. In the E&M Code section, click Calculate Code.
    A calculated E&M code appears in the Calculated code field.

    Fields in the EM Level Summaries and the Detailed EM History panels automatically display the values used in calculating the EM code.

  6. To bill one of the eye codes, select the appropriate code in the Eye Codes section.
    The selected code appears in the Submitted code field.
  7. Click Submit Code.
    The code is submitted to the Superbill and the Submitted message appears.
