Review Patient's History Summary on SOAP

You can review and update a patient's history summary documented by the intake staff during the previous encounters, in the History panel on the SOAP template. The various categories available in the History panel enable you to review the corresponding history records. Open NextGen® Enterprise EHR. Open the SOAP template. In History, select a history category in the left pane, and review the history records. For the Social category, the social history details appearing may vary depending on the patient's age. To edit a history record, select the record in the grid, and update the desired details on the category specific history template. Note: For the Social category, you must select the subcategories to review and update the social history details on the Social History template. To delete a history record, select the record in the specific category, and select Remove. Note: You cannot delete the history records in the Social and Diagnostic categories. You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Patient's History Summary Review by Provider

You can review patient's history summary using the History Summary panel on the Home Page template. The History Summary panel displays the patient history documented by the intake staff and provider on the Intake, Histories, and SOAP templates. The data grid includes a list of categories in the left pane that enables you to view and document the associated data. You can review and update the patient's complete medical history. Patient's History Summary

The History Summary panel on the Intake template enables you to document a patient's past medical or surgical history, interim, social, family, and diagnostic study. It also displays the patient history documented by the Intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by clicking the Add to HPI Grid button. The grid includes a list of categories in the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option. Reviewing Patient's General Information on SOAP

You can view the general details of a patient in the General panel on the SOAP template. In the General panel, you can do the following: Document the relationship of the person who provides the patient information, in the Historian field. Capture the electronic Summary of Care requests and receipts that are completed electronically, and provide the information as reportable fields on the Transitioning Into Care template. On the Addition Visit Information template, you can provide answers for the preconfigured questions in the Interpreter Details category. You must document interpreter details when someone else interprets the patient’s communication for you. Indicate if the patient for the current encounter is new or established.
