Patient's History Summary

The History Summary panel on the Intake-OPH template enables you to document a patient's past medical or surgical history, interim, social, family, diagnostic study, and glucose monitoring history. It also displays the patient history documented by the Intake staff on the Histories template. You can also add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by clicking the Add to HPI Grid button. The grid includes a list of categories in the left pane to select and view the documented data. Categories in History Summary

You can document various history details for a patient in different categories in the History Summary panel. The left pane of History Summary panel on the Intake template includes a list of categories to select and view the documented data. The following table provides a description of each category. Category Description Past Medical/Surgical Enables you to document the patient's past medical and surgical history. You can document the patient history using Add. If no medical or surgical history exists, you can select the No relevant past medical/surgical history check box. You cannot select the check box if medical surgical history present in the grid. Interim Enables you to document the interim history. Social Enables you to document the Social history. You can select an option from the left pane using Add/Edit. Histories Template

You can add, update, delete, and review a patient's history summary details. The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. You can also add skin cancer details. Documenting the Patient History Summary You can add, update, delete, and review a patient's history summary details from the History Summary panel. Generate the Intake Note You can generate the intake note for a patient using Generate Intake Note in the Intake and Histories template. Documenting the Patient History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. Patient's History Summary Review by Provider

You can review patient's history summary using the History Summary panel on the Home Page template. The History Summary panel displays the patient history documented by the intake staff and provider on the Intake, Histories, and SOAP templates. The data grid includes a list of categories in the left pane that enables you to view and document the associated data. Patient History

You can review, add, and update the patient histories. The History panel on the Billable Visit template enables you to select the visit type and review the diagnoses, histories, medications, allergies, and review of systems of a patient. You can also add or update the details of the history of present illness (HPI). The following grids are automatically filled with the data documented on the Intake template. You can add, edit, and remove the data as needed.
