Categories in History Summary

Category	Description
Past Medical/Surgical	Enables you to document the patient's past medical and surgical history. You can document the patient history using Add. If no medical or surgical history exists, you can select the No relevant past medical/surgical history check box. You cannot select the check box if medical surgical history present in the grid.
Interim	Enables you to document the interim history.
Social	Enables you to document the Social history. You can select an option from the left pane using Add/Edit.
Family	Enables you to document the Family history. You can document the details using Add.
Diagnostic	Enables you to document the diagnostic history. You can document the details using Add/Edit. You can view diagnostic studies by Specialty. To view the diagnostic studies relevant to your specialty, you can select the Specialty option. The All option enables you to view the diagnostic studies relevant to all the specialties. You can add a historical diagnostic study created in the current encounter with a result or interpretation to the Reason for Visit grid as a Studies Reviewed record using Add to HPI Grid.

Add Medical and Surgical History

You can add the medical and surgical history of a patient on the Histories template or the History Summary panel. If there is no relevant medical and surgical history for a patient to document, you can select the No relevant past medical/surgical history option on the Histories template or the History Summary panel for the medical/surgical section and skip documenting the details. Open NextGen® Enterprise EHR. To open the Past Medical History template, do one of the following: On the Histories template, select Medical/Surgical/Interim, and select Add. In the History Summary panel on Intake, select Past Medical/Surgical, and select Add. To document a medical condition or surgical procedure on the Past Medical History template, select the condition in the Medical or Surgical panel.Based on the selection, the Onset Date field appears in the Medical panel or the Date field appears in the Surgical panel along with the Details icon beside the selected condition. Documenting the Patient History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Buttons and Check Boxes in History Summary Panel

You can use the buttons and check boxes present on the History summary panel. The following buttons and check boxes are available in the History Summary panel. Button/Check Box Description Edit Enables you to review and make changes in any of the documented patient history data. Add Enables you to add any new patient history by selecting a category in the left pane. Add to HPI Grid Enables you to send a diagnostic study, that was created in the current encounter with a documented Result or Interpretation to the grid in the Reason for Visit section.
