Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields. The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template If the patient is married, enter the spouse name. Open the Patient Demographics Template

You can document patient details such as contact number, address, Primary Care Physician (PCP), primary practice provider, insurance, pharmacy, provider, emergency contact, employment, and reminder details, on the Patient Demographics template. Open NextGen® Enterprise EHR. Select a patient record. To open the Patient Demographics template, do one of the following: Open the Templates module, and double-click Patient Demographics in the Demographics tab. On the Home Page template, select the Demographics link. The Patient Demographics template opens. Patient Demographics Add Patient Demographic Information You can add demographic details of a patient on the Patient Demographics template. Editing the Patient's Address History You can view and edit the patient's address history details in the Address panel of the Patient Demographics template. Patient Demographics

You can review a selected patient's demographic information in the NextGen® Adaptive Content Engine Patient Demographics template. You can also add or edit the default pharmacy, historical addresses, reminders, emergency contacts, and employment. To update other patient information, use the Modify Patient Information window. To add a patient, use the Add Patient Information window. For more information on person and patient search and to view patient information, refer to the Person and Patient Lookup in EHR and the Person and Patient Information sections. Update or Delete a Reminder

You can modify reminder details for a patient on the Telephone Call template. Open NextGen® Enterprise EHR. On the Telephone Call template, expand the Reminder panel. Select a reminder from the grid. To update the reminder details, modify the details, and then select Update. The updated reminder appears in the grid. To delete the reminder details, select Remove. Update a Personalized Reminder

You can update a personalized reminder related to patient on the Personalized Reminder or Orderable template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. Expand the Care Guidelines panel and select the Add/Update Reminders link. The Personalized Reminder/Orderables template opens. Select a reminder listed in the grid. The reminder and its details appear in the fields above the grid. Make any changes to the reminder. Text of the reminder Due date Tasking Select Add/Update. The reminder appears in the grid with updated with information. Select Save & Close or Cancel. Parent topic: Personalized Reminders

Access the Patient Demographics Template

You can also access the Patient Demographics template from Demographics links on various NextGen® Adaptive Content Engine. For more information, go to the NextGen Healthcare Success Community, and download the NextGen® Adaptive Content Engine documentation for the specific template. Open an existing patient in NextGen® Enterprise EHR. For more information, refer to the Person and Patient Lookup in EHR section. Open the Select Template window and select the Demographics tab. Select the Patient Demographics template. For more information on selecting a template, go to the NextGen Healthcare Success Community and download the Templates User Guide for NextGen Enterprise EHR. The Patient Demographics template appears, displaying the selected patient's information. Expand the panels to view the information in them. You can add or edit information in the enabled fields.
