Fields on the Health Promotion Plan Data Set
Field Description Plan Type Enter the type of plan. In run-time, the plan type appears in the picklist launched from the Plan Type field on the Health Promotion Plan template. File Maint. Dx Category Enter a category of diseases set up in NextGen File Maintenance. Label for Value Enter the field label for the dynamic value that you want to display at the top of the Health Promotion Plan panel on the Health Promotion Plan template. Two sets of this field appear on the practice template. Data Table Enter the data table to search the required value. Two sets of this field is available on the practice template. Data Field Enter the data field to search the required value. Two sets of this field is available on the practice template. Result Field Enter the results of a search. Create Customized Health Promotion Plan
On the Data Sets Setup template, select Data Set and select Health Promotion Plan. The existing data set items associated with the Health Promotion Plan data set appear in the grid. Health Promotion Plan data set Select Add.The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, select Diagnosis description to search and select a diagnosis.The associated diagnosis code automatically populates. If applicable, select Provider and select a provider. Enter the details in the necessary fields. Select Save, and then select Close.The newly configured data set item appears in the Data Sets Setup template. Note: To add another item, select Clear For Add in the Template Data Sets Setup – [New Record] template and repeat steps 1 and 2. Document Health Promotion Plans
You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Add a diagnosis. Select Plan and select a plan from the list.Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To turn on the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral checkbox. If required, in the Details field, enter any additional information. If you want to add or update patient educat

Health Promotion Plan
The Health Promotion Plan link on the Intake template opens the Health Promotion Plan window where you can document a Health Promotion Plan, Pain Management, Fall Risk, Screening tools/Follow-Up Plans of Care, and Functional and Cognitive Assessment Status. Add a BMI (Weight Management) Plan
You can use the Health Promotion Plan panel to enter a BMI Plan, Hypertension plan, Depression plan or a customized health promotion plan. Your selection in the Plan field determines the other fields that appear on the panel. For more information, go to NextGen Healthcare Success Community, and download the Family Practice User Guide for NextGen® Adaptive Content Engine. Expand the Health Promotion Plan panel. Select BMI Plan from the list in the Plan field. Click the Diagnosis field to search for and enter a diagnosis. Enter recommendations for Diet, Physical Activity and Referrals in the respective fields. Click Add. The plan details are added to the grid. When you are finished entering BMI data, do one of the following: Click OK to close the window. Select another type of plan in the Plan field and complete the entries for that plan. Open the Template Data Sets Setup Template
The Health Promotion Plan template is packaged with BMI Plan, Depression Plan, and Hypertension Plan. You can also create a customized health promotion plans. Open NextGen® Enterprise EHR. Select File > System \ Practice Template.The Select Template window opens. Select the Practice tab. In Show, select All. Select Data Sets Setup. In the Data Set field, select Health Promotion Plan from the list. Select Add to open the Template Data Sets Setup template, and then select Add.
