Add a BMI (Weight Management) Plan

You can use the Health Promotion Plan panel to enter a BMI Plan, Hypertension plan, Depression plan or a customized health promotion plan. Your selection in the Plan field determines the other fields that appear on the panel. For more information, go to NextGen Healthcare Success Community, and download the Family Practice User Guide for NextGen® Adaptive Content Engine. Expand the Health Promotion Plan panel. Select BMI Plan from the list in the Plan field. Click the Diagnosis field to search for and enter a diagnosis. Enter recommendations for Diet, Physical Activity and Referrals in the respective fields. Click Add. The plan details are added to the grid. When you are finished entering BMI data, do one of the following: Click OK to close the window. Select another type of plan in the Plan field and complete the entries for that plan. Pain Management

You can document pain management information for the patient in the Health Promotion Plan template. Scroll down to find and expand the Pain Management panel. Pain Management panel Enter data in the Pain Assessment/Plan of Care fields. Type any comments or use My Phrases to add plan details in the Follow-up plan of care field. Note: My Phrases accessed from this grid are not NextGen® Orthopedic Suite phrases, they are My Phrases that have been set up in the NextGen® Adaptive Content Engine. When you are finished entering pain management data, either click OK to close the window, or open another panel to document another Health Promotion plan. Parent topic: Health Promotion Plan

Document a Therapy Health Promotion Plan

You can document screening and follow-up plans of care for the following: BMI, depression, and hypertension health promotion plans Pain management and fall risk plans of care Functional status Note: You can customize the Health Promotion Plan template in NextGen® Adaptive Content Engine. Fall Risk

Use the Fall Risk panel to screen patients (65 years or older) for fall risk, and to document a care plan as necessary. Expand the Fall Risk panel. Fall Risk panel Use the fields to document the patient’s answers to questions about falls. Document the follow-up plan of care in the Follow-up plan of care grids for Assistive devices and Balance, strength, and gait training. Vitals

The Vitals section allows you to enter vitals for today’s visit, see historical data and vitals graphs, and easily access the Health Promotion Plan template. Alerts above the grid indicate vital signs that are outside of normal range.
