On the Data Sets Setup template, select Data Set and select Test Dates.The existing data set items associated with the Test Dates data set appear in the grid. Test Dates data set Select Add.The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, select Diagnosis description to search and select a diagnosis.The associated diagnosis code automatically populates. If applicable, select Provider and select a provider from Dbp Rendering Provider. Enter the details in the respective fields. Select Save, and then select Close.The newly configured data set item displays in the Data Sets Setup template. Associate LOINC® Codes with Procedure Code
On the Data Sets Setup template, select Data Set and select LOINC Codes.The existing data set items associated with the LOINC Codes data set appear in the grid. LOINC Codes data set Select Add.The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, select Diagnosis description to search and select a diagnosis.The associated diagnosis code automatically populates. If applicable, select Provider and select a provider from Dbp Rendering Provider. In Code, enter LOINC code. In System, enter the code type. For example, LOINC to indicate that the code is LOINC code. In Procedure Code, enter CPT or NextGen Compendium Test keys. In Description, enter the description of the procedure code. Select Save, and then select Close.The newly configured data set item displays in the Data Sets Setup template. Edit Health Promotion Plan Data Set Items
On the Data Sets Setup template, select Data Set and select Health Promotion Plan.The existing data set items associated with the Health Promotion Plan data sets appear in the grid. Highlight a grid to modify. Select Update.The Template Data Sets Setup – [New Record] template opens. Make the necessary changes in the applicable fields. Select Save, and then select Close. Add PT Modifier 59 Data Set Items
On the Data Sets Setup template, select Data Set and select PT Modifier 59.The existing data set items associated with the PT Modifier 59 data set appear in the grid. PT Modifier 59 data set Select Add.The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, select Diagnosis description to search and select a diagnosis.The associated diagnosis code automatically populates. If applicable, select Provider and select a provider from Dbp Rendering Provider. In Code1, enter the CPT code for the first procedure. In Code2, enter the CPT code for the second procedure. In Modifier, enter the modifier as 59. Note: To bill the secondary service or the procedure that is not a part of the primary service or procedure, the modifier 59 is added. In Code Match 1, enter the primary code with which the modifier 59 is attached and sent for billing. In Code Match 2, enter the secondary code with which the modifier 59 is attached and sent for billing. Select Save,

Remove Health Promotion Plan Data Set Items

View Run-Time Result of Creating Customized Health Promotion Plan
In the run-time Intake template, select Specialty as Family Practice and Visit Type as Office Visit. In the Vital Signs panel, select Add. In the BMI/BSA Calculation section, select the BMI Plan link.The Health Promotion Plan template opens. In the Health Promotion Plan panel, select the drop-down button adjacent to Plan and select the plan.The fields, values, and picklist displayed in the Health Promotion Plan panel are configured using the Health Promotion Plan data set.
