Fields on the Health Promotion Plan Data Set

Field	Description
Plan Type	Enter the type of plan. In run-time, the plan type appears in the picklist launched from the Plan Type field on the Health Promotion Plan template.
File Maint. Dx Category	Enter a category of diseases set up in NextGen File Maintenance.
Label for Value	Enter the field label for the dynamic value that you want to display at the top of the Health Promotion Plan panel on the Health Promotion Plan template. Two sets of this field appear on the practice template.
Data Table	Enter the data table to search the required value. Two sets of this field is available on the practice template.
Data Field	Enter the data field to search the required value. Two sets of this field is available on the practice template.
Result Field	Enter the results of a search. Two sets of this field appear on the practice template.
Lo Value	Enter the low value in a range of values. Two sets of this field is available on the practice template.
Hi Value	Enter the high value in a range of values. Two sets of this field is available on the practice template.
Units Value	Enter the type of units to display. Two sets of this field is available on the practice template.
Diet DBPicklist Name	Enter the custom DB picklist name that should launch when you select the first default field in the Health Promotion Plan panel on the Health Promotion Plan run-time template.
Activity DBPicklist Name	Enter the custom DB picklist name that should launch when you select the second default field in the Health Promotion Plan panel on the Health Promotion Plan run-time template.
Referral DBPicklist Name	Enter the custom DB picklist name that should launch when you select the third default field in the Health Promotion Plan panel on the Health Promotion Plan run-time template.
Other DBPicklist Name	Enter the applicable custom picklists. You can add up to 10 custom picklists.
Header	Enter the labels for the four default fields available in the Health Promotion Plan panel on the Health Promotion Plan run-time template.
Show header (Hide/Inactive)	Enter Hide to hide the applicable default field in the Health Promotion Plan panel on the Health Promotion Plan run-time template. Or, Enter Inactive to inactivate the applicable default field in the Health Promotion Plan panel on the Health Promotion Plan run-time template. You can hide or inactivate any of the four default fields.
Info Popup Name	Enter the name of the information that displays when the user selects the Quality button.
Info popup Description	Enter the description of the information that displays when the user selects the Quality button.
Developer	Supports clinical decisions. This is not a mandatory field. The Developer field can be filled with the developer name.
Funding Source	Supports clinical decisions. This is not a mandatory field. The Funding Source field can be filled with the funding source name.
Reference Updated	Supports clinical decisions. When you add the configuration details in the other fields, Reference Updated automatically populates with the current date.
