Fields in Data Sets Setup to Customize Health Promotion Plan Template

Field	Description
Plan Type	Use this field to label what kind of plan you want. The label displays on the template that opens this particular template.
File Maint. Dx Category	This field is based on a category of diseases set up in File Maintenance.
Label for Value	Use this field to label the fields you want to search. Two sets of this field appear on the template.
Data Table	Use this field to select the data table to search. Two sets of this field appear on the template.
Data Field	Use this field to select the data fields to search. Two sets of this field appear on the template.
Result Field	This field displays the results of a search. Two sets of this field appear on the template.
Lo Value	Use this field to select the low value in a range of values. Two sets of this field appear on the template.
Hi Value	Use this field to select the high value in a range of values. Two sets of this field appear on the template.
Units Value	Use this field to select the type of units to display. Two sets of this field appear on the template.
Other DBPicklist Name	Use this field to select custom lists. Enter up to 10 custom lists.
Info Popup Name	Use this field for the name of the information that appears when the user selects the Quality button.
Info popup Description	Use this field for the description of the information that appears when the user selects the Quality button.

Setting Up the Data Sets Fields

The Data Sets Setup template contains many useful fields. You can use the Data Sets Setup template to retrieve values from the data tables and fields that you select. The Health Promotion Plan template retrieves the most recent value in that field and displays it in the results field with a customized label. You can also enter high and low values, and any value out of range appears in red text. You can choose what units you want to display. Note: You need to enter values in two sets of fields on the template. The following fields appear on the Data Sets Setup template. Create Custom Health Promotion Plans

You can create a custom health promotion plan. Open NextGen® Enterprise EHR. Open the Template Data Sets Setup template. In Plan Type, enter a label for the kind of plan you want. In File Maint. Dx Category, enter a disease category set up in File Maintenance. In Label for Value, enter a label that you want to search. Select the data tables and data fields that you want to retrieve values from. In Lo Value and Hi Value, enter low and high values. In Units Value, enter the type of units to display. In Other DBPicklist Name, select up to 10 custom lists. Note: The Developer, Funding Source, and Reference Updated fields support clinical decisions. When you add the configuration details in the other fields, Reference Updated is automatically filled with the current date. Select Save. Create a Custom Plan

You can create a customized health plan for the patient using the Data Sets Setup template. Open the Data Sets Setup practice template. Select the Data Set field. Select Health Promotion Plan from the list. Select Add to open the Template Data Sets Setup template. Enter a label for the kind of plan you want in the Plan Type field. Enter a disease category set up in File Maintenance in the File Maint. Dx Category field. Enter a label in the Label for Value field for the fields you want to search. Select the data tables and data fields that you want to retrieve values from. Enter low and high values in the Lo Value and Hi Value fields. Open the Template Data Sets Setup Template

The Health Promotion Plan template is packaged with BMI Plan, Depression Plan, and Hypertension Plan. You can also create a customized health promotion plans. Open NextGen® Enterprise EHR. Select File > System \ Practice Template.The Select Template window opens. Select the Practice tab. In Show, select All. Select Data Sets Setup. In the Data Set field, select Health Promotion Plan from the list. Select Add to open the Template Data Sets Setup template, and then select Add. Create Customized Health Promotion Plan

On the Data Sets Setup template, select Data Set and select Health Promotion Plan. The existing data set items associated with the Health Promotion Plan data set appear in the grid. Health Promotion Plan data set Select Add.The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, select Diagnosis description to search and select a diagnosis.The associated diagnosis code automatically populates. If applicable, select Provider and select a provider. Enter the details in the necessary fields. Select Save, and then select Close.The newly configured data set item appears in the Data Sets Setup template. Note: To add another item, select Clear For Add in the Template Data Sets Setup – [New Record] template and repeat steps 1 and 2.
