Document the Patient's Status

You can record the patient's status using the Social History - Status template. Open NextGen® Enterprise EHR. Open the Histories template. In the Social panel, select Statuses, and then select Add/Edit.The Adult Social History template opens. Expand the Statuses panel. In the Demographics section, enter the patient details. Select the Employment Information link, and on the Employment/Occupation template, select Add to create a new record. Select Save, and then select Close. Select the occupational hazards or enter the hazards manually.You can enter up to two hazards. In the Radiation Exposure section, enter the exposure date, radiation type, and the radiation dose and unit.By default, the encounter date appears as the exposure date. Add Education and Leisure Activities Details

You can document the details of education and leisure activities of a patient on the Pediatric Social History – Education template. Open NextGen® Enterprise EHR. To open the Pediatric Social History – Education template, do one of the following: On the Histories template, expand the Social panel, select Education, and then click Add/Edit. In the History Summary panel of the Intake template, click Social, click Education, and then click Add/Edit. Pediatric Social History – Education In the Historian field, enter the relationship of the person who provides the patient information. In the Education section, do the following: Enter the patient's current school name and grade. Indicate if the patient has repeated any grades in school. If you select Yes, then enter the grade and the reason for the repetition. Select the grades the patient received in school. Select the appropriate options for the following: Performing Likes school Truancy Suspended or expelled If you select Yes, then enter t

Open the Histories template. In the Social panel, select Lifestyle and select Add/Edit.The Social History – Lifestyle template opens. In the Home Environment/Safety section, in Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. Select the Fall Risk Plan link to open the Fall Risk section of the Health Promotion Plan template, if needed. To open CQM - Falls Risk , select the Quality button. Note: You can view the information about Falls Risk clinical quality measure from CQM - Falls Risk . Select Save & Close. Document Patient's Fall Risk

You can document a patient's risk of falling. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Fall Risk panel. In Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. In the Assistive devices field, enter the follow-up plan of care information. If the recommendations include an exercise program, enter the details in the Balance, strength, and gait training field. Document Registry-Based Exceptions

You can document a registry based exception. Open NextGen® Enterprise EHR. To open the Exception/Measures template, do one of the following: In the navigation bar, select the Exceptions/Measures link. On the Finalize template in the CPT II Claims Based Measure Reporting panel, select the Exceptions/Measures link. Exceptions/Measures For Display, select one of the following: Provider Measures Exceptions (current template) Exceptions (all) Select a measure in the grid.The details of the selected measure appear in the Measure Details and Exception Management sections. If applicable, select the exception class for the measure. Note: If the measure exists in multiple exception class records, a window appears, where you can select the exception class. Viewing Exceptions and Measures

You can manage exceptions and view measure details on the Exceptions/Measures template. You can open the Exceptions/Measures template from the navigation bar or from the CPT II Claims Based Reporting panel on the Finalize template. You can view the following types of exceptions and measures on the Exception/Measures template: Registry-based measures Procedure coded measures Custom data mapping Exception/Measures For information on how to address specific measures, go to NextGen Healthcare Success Community, and download the guides for NextGen® HQM. Viewing Registry-Based Measures You can view the status and explanation of a selected registry-based measure on the top section of the Exceptions/Measures template.
